May Dove Canady is an awkward, lonely young woman, who suffered a troubled childhood due to her lazy eye. She has very few social interactions, her only "true friend" being a glass-encased doll named Suzie made by her mother and given to May for her birthday with the adage "If you can't find a friend, make one." May works at a veterinary hospital, assisting with surgeries. Her optometrist fixes May's lazy eye, first with glasses, then with a special form of contact lens. As May attempts to be more social, she becomes friends with Adam, a local mechanic. She has a fixation on his hands, which she considers to be the most attractive part of him, and they start dating. Meanwhile, May's lesbian colleague Polly begins to flirt with May. One day while feeling especially low, May remarks that Polly has a beautiful neck. Los Angeles is being terrorized by a masked serial killer whose modus operandi is to break into stores after hours, murder the workers, and take the surveillance tapes so he can use them to relive his crimes. At Steve's liquor store (the main door to which has been boarded up due to an incident with a homeless man) the employees, customers, and a deliveryman named Morty discuss the killer, the $500,000 being offered as a reward for his capture, and how they would kill him, the mentioned methods being eye gouging, exsanguination in plastic wrap, sawing, and decapitation. After the store closes, Steve has a clerk named Donny, and two regulars (a prostitute named Jess, and a phone sex worker named Mona) stay to play cards in the upstairs apartment as a means of keeping Steve's nephew Jimmy (an addict going through drug withdrawal) occupied. Unbeknownst to the quintet, the Convenience Store Killer has chosen them as his next victims, cutting the homeless man (who resides in a nearby alley) in half before sneaking into the building.

On the mainland, five travelers are preparing to tour the islands, and are joined by Julie, who asks for a ride to an island that some friends of hers live on. The only one who objects to this detour to the island (which Julie explains has only a few permanent residents, and only sees tourists a few months out of the year) is Carol, whose tarot cards convince her that something bad will happen if they go to the island. On June 6, six women in a close-knit Amish community go into labor and deliver six girls, sparking fears that they could fulfill an ancient prophecy. The prophecy states that six girls will be born on the sixth day of the six month (6-6-6) and that one of them will become the "Devil’s Hand". Out of fear, one mother kills both herself and her daughter shortly after her birth, and the remaining five girls grow up with relative ignorance of the prophecy. As they come closer to their eighteenth birthday, their actions are constantly monitored by the community, especially Elder Beacon (Colm Meaney), who views any ungodly actions as proof that one of them is Satan's minion. This is all made more troubling by the fact that one of the girls, Mary (Alycia Debnam-Carey), has started having terrifying visions that could suggest that she is the Devil's Hand. As tensions rise, a mysterious figure begins to murder the girls one by one.
