During an orchestrated drug bust at a marine loading dock, Detective Stan Zedkov (Michael Rooker) kills Triad lieutenant Peter Wei (Yau-Gene Chan). Looking to exact revenge for his son's death, crime boss Terence Wei (Kenneth Tsang) sends for professional assassin John Lee (Chow Yun-fat). Paying off an old debt, Lee has already killed two targets for Wei, and the crime boss tells him that this third and final job will settle the obligation. However, Lee's conscience prevents him from completing his final assignment: to murder Zedkov's seven-year-old son Stevie (Andrew J. Marton) before the detective's eyes. Realizing that his actions will result in retaliation against his mother and sister, Lee prepares to return to China, enlisting the help of old friend Alan Chan, a monk in a local Buddhist temple, to make arrangements to have his family moved to a secure location. Infuriated by Lee's disobedience, Wei orders his head lieutenant, Michael Kogan (Jürgen Prochnow), to lead the hunt for Lee, and has his people in China begin the search for Lee's family.

The film begins with a group of news reporters watching, and gossiping, at an elaborate wedding reception for Yoshiko Iwabuchi (Kyoko Kagawa), the daughter of Vice President Iwabuchi (Masayuki Mori, the villain character) of the Unexploited Land Development Corporation, a construction company, and Koichi Nishi, the president's secretary (a bespectacled Toshiro Mifune). The police interrupt the wedding when corporate assistant officer Wada, who is the wedding reception's master of ceremony, is arrested on charges of bribery in a kickback scheme. The reporters comment this incident is similar to an earlier scandal involving Iwabuchi, administrative officer Moriyama, and contract officer Shirai. That earlier case was hushed up after the apparent suicide of Assistant Chief Furuya, by jumping off the corporate office building, creating a dead end in the investigation before any of the company's higher-ups could be implicated.

The film opens with Peppino as a small child singing the popular song “Nel blu, dipinto di blu” with his brother in the back seat of a car on the way to a family gathering. The family is one of good standing in the social community and they are celebrating the fact that they have such a good life. In this scene the relationship between Peppino and his uncle Cesare is established. His uncle is a Don or Mafia boss in the small town of Cinisi where the story is set. In a scene soon after the happy family gathering we see don Cesare blown up by a car bomb which was planted by a rival Mafia boss. This ends Peppino’s time of innocence. Even as a small child he is thrust into the realities of life in the Mafia.

U.S. intelligence agent Jim Carter is sent to Japan as a National Weekly Indicator journalist to find Taro Matsudo who is helping the Communists there. Matsudo happens to be Carter's college friend. In his hotel, Carter meets Steffi Novak, a mysterious woman who speaks six languages and wishes to accompany him. Together they both reach a place where Taro is present. However, he does not want to meet Jim. Back at his hotel, Jim receives a telegram informing him to reach Enoshima island. Here he meets Taro who refuses to divulge any information about his commander. He comes to know that Matsudo aspired to be a kamikaze pilot but when Japan surrendered during World War II he sided with the Communists. When Jim returns to his hotel room, he is beaten by a group of Japanese men who tell him to stay away from Taro.
