In 1978, narcissistic, manipulative Madeline Ashton performs in a musical on Broadway. Madeline invites long-time rival Helen Sharp, an aspiring writer, backstage along with her fiancé, plastic surgeon Ernest Menville. Ernest is smitten with Madeline, soon breaking off his engagement with Helen to marry her. Seven years later, Helen is in a psychiatric hospital after fixating upon Madeline. Obese and depressed, Helen feigns rehabilitation and is released, plotting revenge on Madeline.

In 1985, Connor MacLeod, also known as the Highlander, is in New York City. In an arena parking garage, Connor is confronted by fellow immortal Iman Fasil, and decapitates him, upon which an energy surge destroys several cars around him. Police swarm the exit to the garage, and arrest Connor.
The film takes place in New York City in the year 2095 where genetically altered humans live side by side with unaltered men and women, and where Central Park has been mysteriously encased in an "intrusion zone" where people who attempt to enter are instantly killed. A strange pyramid has appeared over the city; inside, the gods of ancient Egypt have judged Horus, one of their fellow gods, to cease his immortality.
The film opens in a future Paris with scenes establishing the kidnapping of 22-year-old scientist Ilona Tasuiev, who works for the megacorporation Avalon. The focus transitions to police captain Barthélémy Karas, as he defuses a hostage situation by killing the hostage-takers. Afterwards Karas is given the job of solving the mystery surrounding Ilona's disappearance. Karas begins by contacting Dr. Jonas Muller, a former Avalon scientist familiar with her.

The treatments by Dr. Devilers (Alain Delon) make her and the other guests feel better, and for a while that is enough. However, Helene stumbles upon the resort's dark secret: the patients rejuvenation comes at the expense of Portuguese youths, who arrive at the clinic to work in the gardens and are never seen again.
