The film centers around a serial killer known as "The Card Player", who is kidnapping young women in Rome. Using a webcam set-up, the killer challenges the police by forcing them to play hands of Internet poker. If the police lose, the kidnapped victim is tortured and murdered on-screen. When a British tourist is among the girls murdered, policeman John Brennan (Cunningham) is assigned the case and quickly teams up with Italian detective Anna Mari (Rocca). The duo have their work cut out for them when the Police Chief's daughter (Argento) becomes the killer's latest kidnapping victim.

The film starts as Angela sits on a subway. The train halts and passengers are told to evacuate, as a man has just stepped in front of the tracks and died. While being led out of the station, Angela begins to move towards the tracks to see the man's remains. She is warded away at the last instant. Angela is a university student in Madrid, writing a thesis on audiovisual violence in the family. At a thesis meeting, she asks her thesis director, Figueroa, to help her find the most violent videos in the school's library. After class, Angela seeks out the help of a fellow student, Chema, who is known for his collection of violent and pornographic videos. As Angela begins to watch violent films with Chema, Professor Figueroa finds a tape in a hidden hallway of the school's audiovisual archives. The next day, Angela finds Figueroa dead of apparent heart failure in the university's viewing room, a video tape in the player. Angela takes the tape and leaves for class. She later learns that Figueroa died of an asthma attack, and that a younger professor, Castro, will now be directing her thesis project. Angela goes to Chema's house to watch the stolen film, and Chema realizes that this is a snuff film, or a film of someone actually being murdered. As they watch the women being tortured, killed and disemboweled, Chema realizes that the women in the film was named Vanessa, a girl who attended their university and went missing two years ago. Chema and Angela are also able to determine which kind of camera the killer used, and XT 500, based on the quality of its digital zoom, and that the film was shot in someone's garage.

Max Renn (Woods) is the president of CIVIC-TV, a UHF television station in Toronto that specializes in sensationalistic programming. Displeased with his station's current lineup, Max is looking for something that will break through to a new audience. One morning, he is summoned to the clandestine office of Harlan (Peter Dvorsky), who operates CIVIC-TV's pirate satellite dish which can intercept international broadcasts. Harlan shows him Videodrome, a plotless show apparently being broadcast out of Malaysia which depicts the brutal torture and murder of anonymous victims in a reddish-orange chamber. Believing this to be the future of television (seemingly staged snuff TV), Max orders Harlan to begin pirating the program.

The movie begins with Elizabeth logging into a social media site known as "The Den". It is revealed that she is doing this for a project and her goal is to chat with as many strangers as possible. However, things become grim when she starts a chat with someone whose webcam appears to be broken. After having random chats with her, the webcam somehow turns on and a woman is seen being murdered. Throughout this time, Elizabeth's account has also been continuously hacked however she does not realize this. Her friends try to convince her it is a hoax but she remains unconvinced. She asks a friend of hers to hack the account and find out where it is being operated from, but it ultimately doesn't work since the signal is coming from too many locations to be found. Though Elizabeth had contacted the police about the murder, they do not believe her and she remains helpless. Once her boyfriend mysteriously disappears, she panics and calls the police once again. They check his apartment but find it completely empty. Another one of Elizabeth's friends is then shown getting a message from Elizabeth (though it is obviously not actually Elizabeth, as she is with the police). The message tells her friend to come over and she is then shown to be abducted just like Elizabeth's boyfriend. Elizabeth becomes more and more frantic after receiving a message showing a close up of her sister's home and an unknown figure entering it. She calls her sister and tells her to leave, but is too late and her sister and her unborn baby's life is threatened. The attacker then leaves but later returns posing as a police officer. Elizabeth leaves with an officer and contacts her sister once she reaches her apartment to tell her that she will be back with her soon. As Elizabeth is packing she receives another message showing the head detective getting murdered and then another one of her and the inside of her apartment. She goes to open her closet where a hooded figure is, she stabs him repeatedly and flees from her apartment. To her dismay, she is attacked yet again. She later awakens in an unknown building and is chained to a wall. There is a computer in her room on which she receives a video of her boyfriend. He informs her that there are more of them and then is murdered. After overpowering another hooded man in her room she attempts to escape and realizes there are multiple attackers. After finally getting out of the building, she steals a car from one of the attackers but then crashes. Another woman is then seen on the same site Elizabeth was on. She comes across Elizabeth's account where she witnesses Elizabeth being murdered. The scene then cuts to a man putting his credit card number into a site to watch more videos of murders like Elizabeth's but quickly closes the browser when he is interrupted by his young son.
