In the present-day American Midwest, society struggles to function in the aftermath of a zombie pandemic barely under control. Maggie Vogel (Abigail Breslin) calls her father from a broken city under curfew; her voicemail urges that he not seek her and that she loves him. Her arm was bitten. Knowing she has only weeks before the "necroambulist virus" turns her cannibalistic, she left home to protect her family. Maggie's father Wade (Arnold Schwarzenegger) has searched two weeks, despite her warning. Finding her in a hospital for the infected, he brings Maggie home to care for her until she must eventually be quarantined. During their return, a zombie attacks Wade at an abandoned gasoline station and he breaks its neck.

A young female traveler, Charlotte (Émilie Dequenne) gets into a fight with a group of bikers. After this, she meets Max, a hitchhiker. The two stop off at La Spack, a rundown roadside eatery run by a woman (Yolande Moreau), after whom the restaurant is named. The biker gang reappears and is chased off by the owner of the restaurant. Moments later, Max vanishes after heading to the bathroom. Charlotte elects to investigate at nightfall to the spot where Max disappeared. Charlotte later finds herself bound and caged, a prisoner of La Spack, who sees her as the next meal for her brood, a pack of ghouls.

The film opens on a sleepy gas station, where the slack-jawed pump attendant (Charles Baker) is attacked by a seemingly rabid animal. Meanwhile, a young couple, Seth Belzer (Paulo Costanzo) and Polly Watt (Jill Wagner), retreat to the wilderness for a romantic camping trip in the ancient forests of Oklahoma, but their plans are shattered when they are car-jacked by an escaped convict, Dennis Farell (Shea Whigham), and his drug-addict girlfriend, Lacey Belisle (Rachel Kerbs). As the foursome travel the back roads together, they get a flat tire when they run over a small splinter infected animal on the road. Finding shelter at the now abandoned gas station, they fend off larger incarnations of a parasite as it infects more victims.
