Adonis Johnson Creed grew up without knowing his father Apollo, the one-time world heavyweight champion, and struggles to channel the strength, skill and toughness he inherited from him. Eddie Morra (Bradley Cooper), a struggling author suffering from writer's block, living in New York, is stressed by an approaching deadline. After taking the pill, Eddie finds himself able to learn and analyze at a superhuman rate and recall memories from his distant past, with the only apparent side effect being a change in the color of Eddie's irises while on the drug—his eyes becoming an intense shade of electric blue. Under the influence, he cleans his messy apartment and writes ninety pages of his book. The next day, the effects having worn off, he seeks out Vernon in an attempt to get more. While Eddie is out running an errand, Vernon is murdered. Eddie returns, calls the police and then discovers Vernon's NZT stash just before they arrive, taking it for himself. After giving a statement at the precinct, Eddie returns home and begins ingesting the drug daily. With the help of the drug's amazing effects, Eddie spends a few weeks cleaning up his life—finishing his book, getting fit, and making friends with a group of young jet-setters, who take him on vacation to Europe, where he mingles with the rich. During all this, Eddie tests out his enhanced learning abilities; he becomes a proficient piano player in just three days, as well as becoming fluent in several languages.

Philadelphia, 1999. In a violent home invasion, engineer Clyde Shelton (Gerard Butler) is forced to witness the rape and murder of his wife and young daughter by Clarence James Darby (Christian Stolte) and his accomplice Rupert Ames (Josh Stewart). Prosecutor Nick Rice (Jamie Foxx) is unable to use DNA evidence to securely convict both accused. Unwilling to take a chance on lowering his high and yet unbeaten 96% conviction rate, he chooses to make a deal with Darby (the actual murderer), letting him plead guilty to a lesser charge, in return for testifying against Ames. Ames is falsely found guilty of masterminding the break-in and both murders and is sentenced to death. Shelton feels betrayed by Rice's actions, as he had pleaded with him not to make the deal and to at least try convict both of them, and because he chose to release the actual killer. Darby serves his shortened sentence and is released just a few years later.

In New York City's Central Park, people begin committing mass suicide. Initially believed to be caused by a bio-terrorist attack using an airborne neurotoxin, the behavior quickly spreads across the northeastern United States. Elliot Moore, a high school science teacher in Philadelphia, hears about the attacks and decides to go to Harrisburg by train with his wife, Alma. They are accompanied by his friend Julian and Julian's eight-year-old daughter Jess. Julian's wife is stuck in Philadelphia but is expected to meet them in Harrisburg. The train loses all radio contact en route and stops at a small town. They receive word that Philadelphia has been attacked by the toxin and Julian's wife was not able to get on the train to Harrisburg. Julian decides to go look for her, leaving his daughter with the Moores while he hitches a ride. However, the town has already been hit by the toxin. Succumbing to it, the driver and Julian both commit suicide.
