Le film commence en 1922 dans la région de Londres, au Royaume-Uni. Vita Sackville-West est une écrivaine britannique à succès et une femme émancipée qui fume, conduit, signe ses livres de son vrai nom et vit une relation ouverte avec son mari, le diplomate Harold Nicolson, lui-même discrètement bisexuel. Vita vient de lire Mr Dalloway, le dernier livre de Virginia Woolf, écrivaine qu'elle admire et souhaite rencontrer. Mais Vita est issue de l'aristocratie conservatrice, et tant sa mère que son mari se montrent méfiants vis-à-vis de Woolf, qui passe pour folle et fait partie d'un groupe d'écrivains socialistes à la réputation de bohêmes, le Bloomsbury Group, dont les œuvres n'ont à ce moment qu'une diffusion confidentielle. Vita est bisexuelle et, dans sa jeunesse, elle a frisé le scandale en s'enfuyant avec une de ses amoureuses, Violet Keppel. Sa mère, qui a étouffé l'affaire, intervient pour empêcher la parution de son futur livre et lui défend de rencontrer le Bloomsbury Group. Vita finit par se rendre à l'une de leurs soirées et rencontre Virginia : c'est un coup de foudre, non réciproque. Déterminée à mieux connaître Virginia Woolf, Vita entame avec elle une correspondance vivace et soutenue, dont le film utilise de nombreux extraits. Elle s'intéresse à la maison d'édition fondée par Virginia Woolf et son mari Leonard Woolf, Hogarth Press. Vita va jusqu'à lui soumettre et lui dédicacer son prochain livre, Seducers in Ecuador, ainsi que les suivants, qui sont des succès et améliorent les finances de la maison. Peter (Levi Miller) is a young boy who is left as a baby on the steps of an orphanage in London by his mother Mary (Amanda Seyfried), an establishment under the care of Mother Barnabas (Kathy Burke). Several years later, during World War II,The Cold War, upon learning that she is hoarding food for herself, Peter and his best friend Nibs try to steal it to distribute amongst themselves and the other orphans but they get caught. In the process, Peter finds a letter written by his mother, declaring her love and assuring Peter they will meet again "in this world or another".

In early 1920s Copenhagen, the illustrator and artist Gerda Wegener (Alicia Vikander) asks her husband, Einar Wegener (Eddie Redmayne), to stand in for a female model. The popularity of the portraits leads to Gerda painting her husband in further pictures as a woman. Einar develops an attraction for a female physical appearance and begins living as a woman named Lili Elbe. Ultimately Elbe becomes the first ever recipient of male to female sex reassignment surgery, and Gerda supports her decision, although their marriage becomes strained when Gerda comes to the realization that Lili is no longer the person she married. A childhood friend of Lili, Hans Axgil (Matthias Schoenaerts), shows up and forms a complex love triangle with the couple.

The film begins in 2010, with a 65-year-old Howard Marks going onto a stage in front of a packed theatre to great applause. Marks asks if there are any plain-clothes policemen sat in the audience, to which the reply is negative. Then, while lighting up a joint, Marks asks "who here is a dope (marijuana) smoker?", to even greater applause. The film then diverts into Marks' internal monologue as he recounts his life.
