In New York City, Nick Cassidy (Sam Worthington) checks in to the Roosevelt Hotel under the false name of Walker, goes to his hotel room on the 21st floor, and climbs on the ledge, apparently ready to commit suicide. The crowd below calls the police, with Dante Marcus (Titus Welliver) controlling the crowd, while Jack Dougherty (Edward Burns) tries to talk with Nick. However, Nick will only speak to negotiator Lydia Mercer (Elizabeth Banks), who is on a leave of absence, after failing to save a suicidal policeman. Lydia arrives at the hotel room and manages to acquire Nick's fingerprints from a cigarette they share. Dougherty has them analyzed and discovers that Nick is an ex-policeman, arrested for stealing the $40 million Monarch diamond from businessman David Englander (Ed Harris). Nick was given a 25-year sentence but escaped from the Sing Sing Correctional Facility one month earlier, after being allowed to attend his father's funeral. Nick, however, maintains his innocence and accuses Englander of framing him for the theft of the diamond, as Englander lost his fortune and was too proud to sell the diamond.

Family man Abel Plenkov (Raul Esparza), a sufferer of schizophrenia, accidentally discovers that he is the Riverton Ripper, a local, masked serial killer. After killing his pregnant wife, Sarah (Alexandra Wilson), and then his psychiatrist, he is shot down and carted away in an ambulance, leaving his young daughter Leah and premature son orphaned. On the way to the hospital, a paramedic (Danai Gurira) suggests that Plenkov himself is innocent but that he houses multiple souls, with the Ripper's being one of them. Near death, Plenkov unexpectedly revives, slashing the paramedic in the throat, causing the ambulance car to crash and burn and seemingly escapes.

Louis Salinger, of Interpol, and Eleanor Whitman, an Assistant District Attorney from Manhattan, are investigating the International Bank of Business and Credit (IBBC), which funds activities such as money laundering, terrorism, arms trading, and the destabilization of governments. Salinger's and Whitman's investigation takes them from Berlin to Milan, where the IBBC assassinates Umberto Calvini, an arms manufacturer who is an Italian prime ministerial candidate. The bank's assassin diverts suspicion to a local assassin with political connections, who is promptly killed by a corrupt policeman. Salinger and Whitman get a lead on the second assassin, but the corrupt policeman shows up again and orders them out of the country. At the airport they are able to check the security camera footage for clues on the whereabouts on the bank's assassin, and follow a suspect to New York.

The film opens on a black screen, with white letters describing the date and place, as we hear radio chatter about a "suspicious vehicle" followed by the sound of an explosion and gunfire. We then cut to see two Marines driving, wearing dress blue uniforms, to an unmarked house in the middle of the night and knocking on the door. Finally we cut to Lt. Colonel Michael Strobl (played by Kevin Bacon) searching on his computer the casualty report for the Middle East. After a couple clips of Michael running through the woods, service members' coffins getting put into an airplane, and some driving. We find the Lt. Col. at work giving a presentation to several other Marines. Then the Lt. Col. makes his way home with his family for a short while, then the camera cuts to five Marines in woodland MarPat camouflage taking US Flag draped coffins off of an airplane in the rain. After a short clip of the Lt. Col. eating dinner with his family, we find him looking at the casualty report yet again but this time he writes down some information. The movie then cuts to him in an interview with a higher ranking Marine and he asks to escort a Marine named PFC (Private First Class) Chance Phelps. He says it is because the PFC is from his hometown, then we see him discuss his choice with his wife. After he explains that he is doing this only because the PFC is from his hometown and has no other meaning, the film cuts to several people doing medical procedures on a dead body which one can assume to be PFC Phelps. After the morticians are shown, the film then cuts to the Lt. Col. leaving and telling his wife goodbye.

Telly Paretta (Julianne Moore) believes that her son, Sam (Christopher Kovaleski), died fourteen months ago in a plane crash, but her husband Jim (Anthony Edwards) tells her that she's delusional and that they have never had a son, and Eliot (Jessica Hecht) doesn't appear to believe in Sam's existence despite her closeness to him. Dr. Munce (Gary Sinise) tells her that Sam was merely a figment of her imagination and is just imagining a life that might have been. He recommends that she be sent to a hospital, but she runs away and meets with a man named Ash (Dominic West) who she thinks is the father of a girl (Kathryn Faughnan) who was friends with her son and died in the same crash. Victor Rosa (Leguizamo) is a drug dealer in New York who sells a specific brand of heroin called "Empire". His territory is located in the South Bronx, where his other rivals all maintain an uneasy truce maintained because they all purchase their drugs from the same supplier, the drug lord La Colombiana (Isabella Rossellini). Victor is invited to a chic white collar party by his girlfriend Carmen (Delilah Cotto).
