In 500 A.D., Hrothgar, king of Denmark, and a group of warriors chase a large and burly man, whom they consider a troll, and his young son, to the edge of deep cliff. The father directs his young son, Grendel, to hide from the attackers' view; whereupon The Danes shoot the father dead, and his dead body plunges onto the beach far below. The Danish king sees the young Grendel, but spares him. Later, Grendel finds his father's body and cuts the head off to take it home. Many years later, the severed (and mummified) head is inside a cave where the boy Grendel has become as large and powerful as his father, and plans revenge.

The movie takes place after the Vietnam War in 1979 and revolves around Corporal Rudy Spruance (Jason Biggs), a soldier mistakenly sent to Qangattarsa, a U.S. Army Medical Corps base in the Arctic instead of his official base in Hawaii. He soon finds out that his records have accidentally been replaced by the records of Corporal Martin Pederson, who probably has been sent to Hawaii. While he is posted as the base's PIO (Public Information Officer), he finds out that underneath the base, there is a secret and classified hospital ward, where the near-insane commanding officer, Lt. Col. L. G. Woolwrap (Jeremy Northam), has been ordered to keep soldiers from a secret mission led by himself in the Vietnam War who have incurable wounds.

High school teacher and writer Frannie Avery meets a student at a local bar, and when she heads to the bathroom sees a woman performing oral sex on a man. A few days later, Detective Giovanni Malloy questions her as he investigates the gruesome murder of a young woman, whose severed limb was found in her garden. They flirt despite the grisly nature of their introduction, and meet at the same bar later. Frannie is alternately thrilled and frightened by the detective's sexual aggressiveness, even as she grows more disillusioned with the attitudes and crude behavior of other men, including the detective's partner, Richard Rodriguez. Even as Malloy defends his partner, who can no longer carry a gun because he threatened his unfaithful wife, he promises he will do anything she wants except hit her. She leaves abruptly and is assaulted walking home, but calls Malloy and their affair begins that night.

Englar Alheimsins, which has often been called an Icelandic One Flew Over the Cuckoo's Nest, is the story of an Icelandic man named Páll (Ingvar E. Sigurðsson).
