Sarah (Fiona Dourif) is one of five people that developed PTSD after surviving a bloody shooting attack in a small diner by a masked shooter, which had the unfortunate side effect of either worsening pre-existing phobias or developing new ones. Like the others, she sought treatment from Dr. Andover (Robert Englund), a brilliant scientist and doctor that developed a new way of treating fear-related ailments by way of his "Fear Chamber". Initially the treatments seemed to be a success but years later the patients have all returned to the clinic due to their fears re-emerging worse than they had before. After urging from her best friend Megan (Cleopatra Coleman), Sarah returns to the clinic as well. Andover is reluctant to resume treatment because one of his previous patients, Paige (Bonnie Morgan), died as a result of her treatment in the Fear Chamber. He ends up giving in due to both the patients and his assistant Osborn (Felisha Terrell) insistence that this is the only way to solve their problems. However he finds that his reluctance may have been founded as one of the patients, Caylee (Angelina Armani), vomited a black substance that gives Andover a strange vision once he touches the fluid. The vision shows a monstrous version of Paige that states that Andover's treatments opened a passage for a sinister being to come over and that all that it needs to finish its travels is for Andover to resume treatment and "open the door". Realizing that this is not Paige but the creature itself, he is shocked back to consciousness.

The film starts in a room full of bomb making material and newspaper articles about LTTE bombings in Sri Lanka, the screen then cuts to a typical busy morning in Colombo. "The Man" (Ben Kingsley) plants five bombs around the city; on a public bus, in a shopping mall and the Polgoda police station, on an intercity train, and at the Katukurunda Airfield. The man then establishes his mini control station on top a skyscraper in Dehiwala and calls the police chief Morris Da Silva (Ben Cross) and informs him that if four prisoners are not released, the bombs will be detonated.

A group of young adults – Oscar and Karen, Beth and her marine infantry boyfriend Randy, and the latter's sister Emily and her boyfriend Niles who is an ex-Navy SEAL – are traveling through the backwoods town of Fort Collins, Louisiana when they pull over at a rundown gas station. An elite group of soldiers led by Mack (Cuba Gooding, Jr.), are sent by a CIA agent named Elissa Cardell (Valerie Cruz) to rescue her father, a scientist named Wesley (Ron Perlman), who is working on an archaeological dig site in the Middle Eastern desert. When they enter the dig site, which is a large underground laboratory, they encounter a priest who has strange boils all over his body. The team's medic, Doc (Taryn Manning) sedates the priest. Click (Brandon Fobbs) uses the dig site's terminal to activate the elevator and notices something codenamed the Gehenna Project and asks Cardell if she knows what it is but she denies knowing about it. Mack orders Nickels (Zack Ward) to stay and watch the priest while the rest of the team takes the elevator deeper into the dig site.

The film takes place at prestigious private school, Dartmoor Academy, where the uptight principal, Morgan Brinway (B. D. Wong) is forcing the second-graders to study opera appreciation. Five feisty kids (leader Sonny, contraption making Loaf, movie loving Domino, sympathetic albeit tough Lucy, and the reluctant participating Witz), nicknamed the "Stinkers" by Mr. Brinway, are secretly skipping class to create chaos on the school grounds. Witz becomes the test pilot for a flying chair, one of Sonny's newest ideas. The Stinkers go to Groundskeeper Roy's (Bronson Pinchot) shed and they steal his leaf blower. Before that, they stole Mr. Brinway's desk chair for the experiment to work. While they are getting ready to test the flying ride, the Stinkers fail to notice that the leaf blower was not entirely duct taped onto the desk chair. When turned it on, the leaf blower flew off the contraption but left Witz sitting in the chair. The leaf blower was heading straight for Mr. Brinway's new convertible, but luckily the leaf blower ran out of gas before it destroyed the car (during the chaos, Mr. Brinway learned of the Stinker's plan and tried to intervene). However, Roy accidentally obliterates the car's side door with his lawn mower blade, causing several thousand dollars of automobile body damage. The Stinkers are then taken to Mr. Brinway's office for punishment. Although he warns them that they would be expelled if they mess up one more time, they soon rebel while trying to keep a low profile and are involved in even more zany misadventures.

The movie opens in the Caribbean, on the Island of San Moreno, where a trio of important looking men (a Chinese militant, a Russian General, and a European businessman) are meeting with the island's governor. In 1983, Mason Storm, a Los Angeles police detective, investigates a mob meeting that takes place by a pier. He records a shadowy figure who assures the mob they can rely on his political support.
