The film opens with a raid on an illegal casino performed by a group of blacks with automatic weapons. The audience discovers the perpetrators are white disguised as blacks. The story moves to a Vietnam War flashbacks with a patrol being ambushed by the Vietcong, and the resulting firefight. The leader of the vigilante veterans is a man known simply as the Lieutenant who was their platoon leader and disfigured in the action. Recuperating in a hospital he regroups the survivors for a series of escalating raids to not only to enrich themselves, but to wipe out organised criminal gangs involved in illegal gambling and narcotics distribution.

Arthur Bishop (Charles Bronson) is a "mechanic" — a top hit man (assassin). He works exclusively for a secret international organization, which has very strict rules. Bishop is very sophisticated, as he regularly listens to classical music, has an art collection, and is a connoisseur of fine wines. However, he is forced to live alone - he cannot show emotions or trust people. Bishop is under constant emotional pressure, so much so that he is prescribed medication for depression, and one day he is temporarily hospitalized when he loses consciousness as a result of the stress. Bishop pays a call girl (Jill Ireland) for an ongoing girlfriend experience to have a simulated romantic (social and sexual) relationship, including her writing him fake love letters.

L'action se déroule à Medicine Bend, une bourgade sur les hauteurs montagneuses du Dakota. Attaquée par des Indiens, la personne chargée du télégraphe a le temps, avant sa mort, d'envoyer un message au fort McClelland. Le colonel Collingwood, le commandant du fort, envoie sur place une escouage dirigée par trois amis : les sergents Merry, Deal et Barrett. Un ancien esclave noir, qui souhaite s'engager dans l'U.S. Cavalry, les suit malgré l'interdiction qui lui en a été signifiée.
