Avril 1945. L'Allemagne nazie, en plein chaos, vit ses dernières heures avant de capituler. Robin Wright plays a fictional version of herself, an aging actress with a reputation for being fickle and unreliable, so much so that nobody is willing to offer her roles. Her son, Aaron, suffers from Usher syndrome that is slowly destroying his sight and hearing. With the help of Dr. Barker (Paul Giamatti), Robin is barely able to stave off the worst effects of her son's decline. Robin agrees to sell the film rights to her digital image to Miramount Studios (a portmanteau of Miramax and Paramount) in exchange for a hefty sum and the promise to never act again. After her body is digitally scanned, the studio will be able to make films starring her, using only computer-generated characters.

In the 1960s Polish People's Republic, Anna, a young novice nun, is told by her prioress that before she takes her vows she must visit her aunt, Wanda Gruz, who is her only surviving relative. Anna travels to visit her aunt Wanda, a chain-smoking, hard-drinking, sexually promiscuous judge who reveals that Anna's actual name is Ida Lebenstein. Ida's parents had been Jews who were murdered late in the German occupation of Poland during World War II (1939–45). Ida was then an infant, and as an orphan she'd been raised by the convent. Wanda, who'd been a Communist resistance fighter against the German occupation, had become the state prosecutor "Red Wanda" who sent "men to their deaths". Wanda's role alludes to "the political show trials of the early 1950s, when Poland’s Communist government used judicial terror (among other methods) to consolidate its power and eliminate its enemies.

At the Bastøy prison for youths, the newest arrival, Erling "C19," soon becomes friends with Olav ("C1"). Under the rule of Håkon, Erling comes to terms with the harsh winter, the mistreatment by the staff, and the hated Housefather Bråthen. Later, Olav sees Bråthen molesting "C5", Ivar, a timid, shy boy in the laundry room. When he raises this with the Governor, the boys are severely punished and C5 is reassigned to duties far away from Bråthen. However, after hearing that Bråthen expects him to be back in the laundry room tomorrow Ivar drowns himself in the freezing Norwegian waters, while the rest of the boys are chopping wood. Bråthen is apparently fired, to the overwhelming delight of the other boys. Olav is paroled and is planning to leave, but on the way to the departing boat, walks past Bråthen, who had been sent away on a shopping trip until Olav, the only witness to Ivar's rape, had left Bastøy. Olav and Erling attack Bråthen and as punishment they are locked in the freezing solitary confinement. They are freed by Bjern, one of the few Bastøy boys who has remained on the island as a caretaker. Olav again pursues Bråthen, and the attack inspires a mass uprising. The boys ransack the prison, and drive away the staff. Bråthen is repeatedly hung and released and then beaten. The barn he is in is set ablaze, but Erling drags him to safety. The navy is brought in and violently puts down the uprising. Olav and Erling manage to escape across a frozen fjord back to the mainland, but Erling falls through a gap in the ice close to shore and quickly freezes to death.

The film opens to the sound of a gramophone playing Axxon N, "the longest-running radio play in history". Meanwhile, a young prostitute, identified in the credits as the "Lost Girl", cries while watching television in a hotel room, following an unpleasant encounter with her client. The Lost Girl’s television displays a family of surrealistic anthropomorphic rabbits who speak in cryptic statements and questions. Occasionally, there are laugh track responses within these Rabbit scenes. These three elements become recurring motifs throughout Inland Empire.

While living with her grandmother in Poland, a young woman, Mariola (Anna Cieslak) falls in love.
