Ingrid and Gregorio Cortez (Carla Gugino and Antonio Banderas) are rival spies who fall in love with each other. They eventually retire and have two children, Carmen (Alexa Vega) and Juni (Daryl Sabara). They currently work for the Organization of Super Spies (OSS) doing desk work. Content with the safety of their family, the two still yearn for field work. Meanwhile, the children think their parents are boring and have no idea of their previous work. Eventually, Ingrid and Gregorio are called back into the field when several agents go missing. Gregorio thinks that a kids' TV show star Fegan Floop (Alan Cumming) has kidnapped the agents and mutated them into his "Fooglies", creatures on his show that speak in gibberish.

College students Paxton (Jay Hernandez) and Josh (Derek Richardson) are traveling across Europe with their Icelandic friend Óli (Eyþór Guðjónsson). In Amsterdam, the three meet a man named Alexei (Lubomir Bukovy), who convinces them to visit a hostel in Slovakia filled with beautiful women. The three board a train to Slovakia, where they encounter a strange Dutch businessman (Jan Vlasák). When they arrive at the hostel, they are greeted by Natalya (Barbara Nedeljáková) and Svetlana (Jana Kaderabkova), who invite them to the spa, and later to the disco. That night, Paxton and Josh sleep with Natalya and Svetlana, while Óli sleeps with the desk girl, Vala (Jana Havlickova). The next morning, they are surprised to see that Óli hasn't returned, and they are unable to contact him. They are later approached by a Japanese girl named Kana (Jennifer Lim), who shows them a photo of Óli and her friend Yuki (Keiko Seiko), who has strangely disappeared as well.

Famed novelist Paul Sheldon (James Caan) is the author of a successful series of Regency romance novels featuring a character named Misery Chastain. Wanting to focus on more serious stories, he writes a manuscript for the final Misery novel. While traveling from Silver Creek, Colorado to his home in New York City, he is caught in a blizzard and his car goes off the road, rendering him unconscious. Paul is rescued by a nurse named Annie Wilkes (Kathy Bates), who brings him to her remote home. When Paul regains consciousness he finds himself bedridden, with both his legs broken as well as a dislocated shoulder. Annie claims she is his "number one fan" and talks a lot about him and his novels. As a reward for saving him, Paul gives Annie his new manuscript which she saved from the wreckage. While feeding him, she is angered and spills soup on him but regains control and apologizes. She buys a copy of Paul's most recently published book, Misery's Child, giving glowing praise to Paul as she progresses through the book. However, when Annie discovers that Misery dies at the end of the book she flies into a rage, almost smashing a table on Paul's head. She reveals that she lied about calling his agent and the authorities; nobody knows where he is. Annie leaves and Paul tries to escape from his room, but she has locked the door.

Picking up after the events of the original film, the people of the small town of Newt, Texas, led by Mayor Burt Hartman (Paul Rae), burn down the farmhouse of the Sawyer family for their role in aiding Jedidiah "Jed" Sawyer (Dan Yeager), also known as "Leatherface", in several murders. The arsonists are celebrated as heroes in the community, and the entire family is presumed dead. However, an infant, Edith Sawyer, is found by one of the townsmen, Gavin Miller (David Born), who promptly murders her mother, Loretta Sawyer (Dodie Brown), and adopts Edith with his wife Arlene (Sue Rock). Twenty years later, Edith has been raised by Gavin and Arlene as Heather Miller (Alexandra Daddario). One day, Heather is notified that her estranged grandmother, Verna Carson (Marilyn Burns), has passed away and left everything to her. Two brothers, Seth and Richard "Richie" Gecko, having just robbed a bank, stop at a liquor store to pick up a map. When the arrival of Texas Ranger Earl McGraw threatens their getaway, they kill him and the cashier, burning down the store in the process. During the gunfight, Richie is shot in the hand. Fleeing a combined force of FBI and local police, they head towards Mexico where a contact has arranged a safe-house for them. Along the way they stop at a motel and unload a bank teller whom they are holding hostage. While Seth goes out to "sight see" checking on the border and to buy fast food burgers, Richie brutally rapes and murders the teller. Seth, who pictures himself as a professional thief, becomes furious over Richie's sadistic behavior.
