Twenty years after the events of Halloween and Halloween II, on October 29, 1998, Marion Chambers (Nancy Stephens), Dr. Sam Loomis' former colleague, returns to her house in Langdon, Illinois, to find it has been burglarized. Her teenage neighbor Jimmy (Joseph Gordon-Levitt) searches the house and finds nothing. While waiting for the police in her house, Marion discovers a file is missing, the one on Laurie Strode (Jamie Lee Curtis), Michael Myers' sister who escaped the attacks two decades earlier. She also realizes that someone has been in her house and immediately rushes back over to Jimmy's house, where she finds him and his friend dead. Michael Myers (Chris Durand) appears, and attacks Marion before slitting her throat, killing her. The police arrive as Michael leaves the house with Laurie's file.

The day after the events in the previous film, Jason, seriously injured after his encounter, goes to a lakefront store to find clothes and a replacement mask. While there he kills owners Harold (meat cleaver in the chest) and Edna (stabbed in the back of the head with a knitting needle) before moving on to Higgin's Haven, a local farmhouse. At the same time, Chris Higgins and her friends return to Higgin's Haven to spend the weekend. Dave, Mike, Cyril, and Moocher are four working-class friends, living in the college town of Bloomington, Indiana. Now turning 19 years old, they all graduated from high school the year before and are not sure what to do with their lives. They spend much of their time together swimming in an old abandoned water-filled quarry, but also often clash with the more affluent Indiana University students in their hometown, who habitually refer to them as "cutters", a derogatory term for locals stemming from the local Indiana Limestone industry and the stonecutters who worked the quarries. Dave is obsessed with competitive bicycle racing, and Italian racers in particular, because he recently won a Masi bicycle. His down-to-earth father, a former stonecutter who now operates his own used car business (sometimes unethically), is puzzled and exasperated by his son's love of Italian music and culture, which Dave associates with cycling. However, his mother Evelyn is more understanding.

Mari Collingwood plans to attend a concert with her friend, Phyllis Stone, for her 17th birthday. Her parents, Estelle and John, express concern at the band and at Mari's friendship with Phyllis, but let her go and give her a peace symbol necklace. Phyllis and Mari go to the city for the concert. On the way, they hear a news report on the car radio of a recent prison escape, involving criminals Krug Stillo, a rapist and serial killer; his son Junior; Sadie, a promiscuous psychopath and sadist; and Fred "Weasel" Podowski, a child molester, peeping tom, and murderer. Before the concert, Mari and Phyllis encounter Junior when trying to buy marijuana. He leads them to an apartment where they are trapped by the criminals. Phyllis tries to escape, then reason with the criminals, but she fails and is gang-raped by Krug, Weasel and Sadie. Meanwhile, Mari's unsuspecting parents prepare a surprise party for her.

The film begins in Leadville, Colorado. Trevor and his girlfriend, Nina, are in an abandoned building having a romantic encounter. They then head back to their respective parents' homes. They see a long line of cars at a military roadblock sealing off the city. An angry man yells at the officers because his child is sick and needs medical care. There has been an outbreak and medical help is needed throughout town. They are introduced to Captain Rhodes and Corporal Sarah Cross. Soon after, privates Bud Crain and Salazar show up and Bud immediately falls in love with Corporal Cross. Cross takes Bud to go with her while she visits her sick mother. When she arrives, Sarah talks to her brother (Trevor) and checks in on her mother. She then goes to see Trevor's friend, Kyle, after she learns his symptoms are similar to her mother's.

Ten years after the Civil War has ended, the Governor of Texas asks Leander McNelly (Dylan McDermott) to recommission a company of Rangers to help uphold the law along the Mexican border. In 1939, Captain Daniel McCormick (Mel Gibson) is a United States Army Air Corps test pilot.
