In this installment of the series Jack Kelly tries to exterminate a major drug cartel after one of the students he is training in the martial arts dies of a suspected drug overdose. Kelly immediately leaps into action by uncovering a cache of weapons he saved from his marine days, and busting into a drug stronghold, guns blazing. Alas, after this battle he is arrested but saved from jail by the quick thinking of his lawyer, who gets him into an insane asylum. Here he will meet many "crazy" characters who will both assist and hinder his quest for justice. After escaping, Kelly continues his fight and eventually discovers that the drugs are laced with rat poison. After some more fighting he uses the drugs to kill the top men of the drug operation.

In a brief scene in 1964, an aging, overweight Italian American, Jake LaMotta (Robert De Niro), practices a comedy routine. The rest of the film then occurs in flashback. In 1941, LaMotta is in a major boxing match against Jimmy Reeves, where he received his first loss. Jake's brother Joey LaMotta (Joe Pesci) discusses a potential shot for the middleweight title with one of his Mafia connections, Salvy Batts (Frank Vincent). Some time thereafter, Jake spots a 15-year-old girl named Vickie (Cathy Moriarty) at an open-air swimming pool in his Bronx neighborhood. He eventually pursues a relationship with her, even though he is already married. In 1943, Jake defeats Sugar Ray Robinson, and has a rematch three weeks later. Despite the fact that Jake dominates Robinson during the bout, the judges surprisingly rule in favor of Robinson and Joey feels Robinson won only because he was enlisting into the US Army the following week. By 1947, Jake marries Vickie.

The mysterious owner of a New York costume store (Astaire) sells Santa suits to three different men. He later shows up in various roles to help the men change their lives. (chauffeur, policeman, jeweler, hot dog vendor, taxi driver, floor walker, and choral director) Bob Willis (Burghoff) is a math teacher who is madly in love with a young fashion model and prominent figure in the disco scene, Polly Primer (Buckman). He hopes to impress her by showing up in a santa suit, but withdraws when he learns of Rod's marriage proposal to her. Polly is not sure to marry Rod and hints to Bob that she longs for being swept away by another guy, but Bob is too shy to act on it. Realizing he would lose her forever to Rod, he interrupts her at a fashion show and tells her he loves her. Polly then reveals having fallen in love with him ever since he first spoke about math, and she accepts his marriage proposal.
