On May 18, 1978, Texas Sheriff John Quincey Wydell and a large posse of State Troopers issue a Search and Destroy mission on the Firefly family for over 75 homicides and disappearances over the past several years. The family arms themselves and fire on the officers. Rufus is killed and Mother Firefly (Leslie Easterbrook) is taken into custody, while Otis and Baby escape. They steal a car, and after killing the driver, they go to Kahiki Palms, a run-down motel.

On October 30, 1977, Jerry Goldsmith, Bill Hudley, Mary Knowles and Denise Willis are on the road in hopes of writing a book on offbeat roadside attractions. When the four meet Captain Spaulding, the owner of a gas station and "The Museum of Monsters & Madmen", they learn the local legend of Dr. Satan. As they take off in search of the tree from which Dr. Satan was hanged, they pick up a young hitchhiker named Baby, who claims to live only a few miles away. Shortly after, the vehicle's tire bursts in what is later seen to be a trap and Baby takes Bill to her family's house. Moments later, Baby's half-brother, Rufus, picks up the stranded passengers and takes them to the family home.

In Salem, Massachusetts, Heidi (Sheri Moon Zombie), a recovering drug addict, works as a DJ at a hard rock station with her co-workers Whitey (Jeff Daniel Phillips) and Herman (Ken Foree). She receives a strange wooden box containing an album by a band named The Lords of Salem. At her home, an old downtown apartment house, she and Whitey listen to the record, which is a series of ancient string and woodwind instruments playing several notes then repeating itself.. Heidi has a vision of witches birthing a baby then damning it. The vision stops once Whitey turns the album off.

The film follows the adventures of El Superbeasto (Tom Papa), a suave, yet violent exploitation film actor/director and former masked wrestler, and his sultry "sidekick" and sister, the super-agent Suzi-X (Sheri Moon Zombie), as they prevent the evil Dr. Satan (Paul Giamatti) from taking over the world by marrying the foul-mouthed stripper Velvet Von Black (Rosario Dawson) with the mark of the devil on her backside. In a flashback, Deborah Myers (Sheri Moon Zombie) visits her son, a young Michael Myers (Chase Wright Vanek), at Smith's Grove Sanitarium. She gives him a white horse statuette as a gift. Michael says that the horse reminds him of a dream he had of Deborah's ghost, dressed in all white and leading a horse down the sanitarium halls toward Michael, telling him she was going to bring him home. Fifteen years later, after having shot an adult Michael (Tyler Mane), Laurie Strode (Scout Taylor-Compton) is found wandering around in a state of shock by Sheriff Brackett (Brad Dourif), who takes Laurie to the emergency room. Meanwhile, the paramedics pick up the Sheriff's daughter and Laurie's friend Annie (Danielle Harris) and Michael's psychiatrist Dr. Loomis (Malcolm McDowell), who are still alive after having been attacked by Michael, and take them to the hospital. Presumed dead, Michael's body is loaded into a separate ambulance. When the driver has a traffic accident, Michael awakens and escapes the ambulance, walking toward a vision of his mother dressed in white and leading a white horse.
