The film starts with a terrified mother and her young son. As she packs a suitcase, she hears something coming towards the door and makes her son hide under the bed. The door bursts open and a supernatural force throws the mother against the wall, killing her. The son flees and runs into his terrified sister. The boy hides under a table, but his sister is caught and pushed over the banister of the stairs, then dragged down to the cellar. The boy hides but is eventually found by the attacker.

Blind since the age of two, 20-year-old Hong Kong classical violinist Mun undergoes an eye cornea transplant after receiving a pair of new eyes from a donor. Rose Elliot (Miracle), a poet living alone in New York City, finds an ancient book called The Three Mothers. It tells of three evil sisters who rule the world with sorrow, tears, and darkness. The book, written by an architect named Varelli, reveals that the three dwell inside separate homes that had been built for them by the architect in Rome, Freiburg, and New York. Rose suspects that she is living in one of the buildings and writes to her brother Mark (McCloskey) in Rome, urging him to visit her. Using clues provided in the book as a guide, Rose searches the cellar of her building and discovers a hole in the floor which leads to a water-filled ballroom. She accidentally drops her keys and enters the water to find them. After she reclaims the keys, a putrid corpse suddenly rises from the depths, frightening her.

In Thailand, young Ton Chatree (Trairat) is sent to a boarding school by his father to get good grades and does not tell his mother about his father having an affair. Marine biology student Beth and Harry are a young couple seeking a distinctive holiday trip, and they spend ten days on a deserted and idyllic coral island on the Great Barrier Reef in Australia, recording many of their adventures with a camcorder. The first two days manifest that they are not alone and the island has a disturbing history. Haunted by a presence, they find an abandoned grave next to an old shack, the latter which has an old drawing and seven tally marks inside on the wall. When Beth and Harry sift through their camera, they discover they were filmed sleeping. Alarmed, Beth wants to leave, but Harry intends to stay, believing kids are playing pranks. They sight fishermen on a boat with guns and later realize that the grave has been remodeled, now decorated with red coral, with a wooden cross embedded at the head of the grave that reads "Coral" on it.
