Anna Quarrels (Rhonda Griffin), librarian in the Rare Books Room, is approached by Mr. Jamison from the University of Chicago who wishes to study Mary Shelley's original manuscript for "Frankenstein; or, The Modern Prometheus". After he's finished, Anna is about to return the manuscript to the stacks when she discovers that Jamison has switched blank paper for the manuscript and walked out of the library with it. Investigating, she learns he used a fake I.D. to access the Rare Books Room and so hires Private Detective, David Raleigh (Justin Lauer) to track him down. David finds fingerprints on the sign-in sheet and discovers the man claiming to be Jamison is really Dr. Winston Berber, an unscrupulous scholar with doctorates in Physics, Mathematics, Folklore, and Philosophy. Berber (Bill Moynihan) is meanwhile gloating over his collection of rare manuscripts; along with the Shelley manuscript, he has obtained the originals of Guy Endore's "The Werewolf of Paris" (1933) and James Putnam's "Mummy" (1993). He now seeks the first edition of Bram Stoker's "Dracula" (1897) to complete his collection. Berber has invented an "Archetype Inducer" and plans to use the manuscripts to bring the four greatest monsters from horror history to life to do his bidding.

Nothing much happens in the lives of 20-something pals Dexter and Royce except for getting high and hanging out with Royce’s girlfriend, Matilda ('Mattie'). This all changes one evening in Northern Ontario town Weedsville when Mattie overdoses on a stash of Dexter and Royce’s drugs — drugs fronted by local drug kingpin and tough-guy Omar to sell in order to cover their previous drug debt. The film is set in Bailey Downs, a suburb where a rash of dog killings has been occurring. Brigitte and Ginger Fitzgerald are teenage sisters who harbor a fascination with death and, as children, formed a pact to die together. One night, while on the way to kidnap a dog owned by school bully Trina Sinclair, Ginger begins her first period, which results in the girls being attacked by the creature responsible for the maulings. The creature wounds and bites Ginger, but Brigitte rescues her. As the girls flee, the creature is run over by a van belonging to Sam MacDonald, a local drug dealer. Some years have passed since the events of the first film. Val has moved away and married Rhonda, while Earl has squandered his fortune on a failing ostrich ranch. He is approached by Carlos Ortega, who informs him that graboids are killing his workers at his oil field in Sonora, Mexico. Earl initially refuses his assistance, but Ortega's taxi driver, Grady Hoover, convinces Earl to change his mind and joins Earl in the hunt, with the oil company paying Grady full fees for his services, as well. Upon arrival in Sonora, Earl meets geologist Kate Reilly, and her assistant Julio, who are scientifically investigating the graboids.

Set 10 years after Return of the Living Dead 3, the film starts out with Charles Garrison (Peter Coyote) traveling to Chernobyl to collect the last 6 canisters of Trioxin 5. Two Russian sellers take him to an empty power station which contains the Trioxin. One of the Russians get Trioxin on his hands, turning him into a zombie. Upon hearing screams, Garrison runs to see the zombie eating the other Russian partner. Garrison promptly shoots the zombie in the head.

The film opens a year after the previous film with Charles Garrison (Peter Coyote) arriving at a mortuary with one of the canisters of Trioxin that he salvaged from the previous movie. He is greeted by a team of Russian government officials whose goal is to destroy the last of the canisters to avoid another incident like that of what happened in Necropolis. Nevertheless, one of them sprays three corpses with the gas, and revives three corpses. Charles, is killed during the incident along with the mortuary owner and one of the government officials.
