Le docteur Eigil Boerne est fiancé à Hélène, qui pour célébrer l'anniversaire de son ami, le convie dans un cabaret où Eigil tombe amoureux de Lily, une jeune danseuse. Lily et Eigil se marient et vont vivre dans un petit village de pêcheurs. Le couple se familiarise avec un peintre devenu aveugle. Eigil l'opère et l'artiste recouvre la vue. Pour mieux suivre la guérison, le médecin décide de l'héberger chez eux, en dépit des remontrances de Lily qui ne voit pas d'un bon œil cette présence. Eigil, ayant appris qu'à la suite de leur rupture Hélène est tombée malade, décide de lui rendre visite. À son retour, il découvre que sa femme Lily et le peintre sont devenus amants et s'enfuit.

Conrad Veidt plays Dr. Warren (the Dr. Jekyll character) who changes into Mr. O'Connor (a parallel of Mr. Hyde). This transformation is brought about, not by experimentation with chemicals as in Stevenson's original, but through the supernatural agency of a bust of Janus (the Roman god of the doorway), which Warren / O'Connor purchases in the opening sequence as a gift for his sweetheart, Jane Lanyon (Margarete Schlegel). When she refuses the gift, horrified, Warren / O'Connor is forced to keep the statuette himself.

A company of men meet for a hunt lasting several days at Castle Vogelöd, being hosted by Lord von Vogelschrey, but a sweeping rain brings the pleasure to nought and they spend their time in the inward parts of the castle. The film is told in an extended flashback. Lorenz Lubota (Alfred Abel), is a clerk in a minor government office, an aspiring poet, and a member of a family headed by a worrisome mother who has a tense relationship with a daughter, Melanie, whom the mother believes works as a prostitute. One day, while Lorenz is walking to work, a woman (Lya De Putti) driving two white horses hits him in the road, knocking him to the ground. Thomas Hutter lives in the fictitious German city of Wisborg. His employer, Knock, sends Hutter to Transylvania to visit a new client named Count Orlok. Hutter entrusts his loving wife Ellen to his good friend Harding and Harding's sister Annie, before embarking on his long journey. Nearing his destination in the Carpathian mountains, Hutter stops at an inn for dinner. The locals become frightened by the mere mention of Orlok's name and discourage him from traveling to his castle at night, warning of a werewolf on the prowl. The next morning, Hutter takes a coach to a high mountain pass, but the coachmen decline to take him any further than the bridge as nightfall is approaching. A black-swathed coach appears after Hutter crosses the bridge and the coachman gestures for him to climb aboard. Hutter is welcomed at a castle by Count Orlok. When Hutter is eating dinner and accidentally cuts his thumb, Orlok tries to suck the blood out, but his repulsed guest pulls his hand away.

Jannings' character, the doorman for a famous hotel, is demoted to washroom (restroom) attendant, as he is considered too old and infirm to be the image of the hotel. He tries to conceal his demotion from his friends and family, but to his shame, he is discovered. His friends, thinking he has lied to them all along about his prestigious job, taunt him mercilessly while his family rejects him out of shame. The man, shocked and in incredible grief, returns to the hotel to sleep in the washroom where he works. The only person to be kind towards him is the night watchman, who covers him with his coat as he falls asleep.
