Le contrat d'un client américain pour la publicité de la machine à laver « Floc » fait l'objet d'une vive compétition entre deux sociétés, représentées par Jean-François pour l'une et par Catherine pour l'autre. Ils recherchent, au Portugal, la jolie lavandière susceptible de venir à Paris pour contribuer au lancement de l'appareil ménager. Une course de vitesse s'engage donc entre les deux équipes qui s'emploient à éliminer la concurrence. Si Catherine réussit à trouver la lavandière, Mariana, c'est néanmoins Jean-François qui parvient à décider la jeune fille à le suivre à Paris en lui promettant de l'épouser.

Conrad Birdie (Jesse Pearson), a popular rock and roll star, receives an Army draft notice, devastating his teenage fans across the nation. Hansel Schmidt is an East German "slip of a girly boy" who loves rock music, and is stuck in East Berlin until he meets Luther Robinson, an American soldier. Luther falls in love with Hansel and the two decide to marry. This plan will allow Hansel to leave communist East Germany for the capitalist West. However, in order to be married, the couple must consist of a man and a woman. Hansel's mother, Hedwig, gives her child her name and passport and finds a doctor to perform a sex change. The operation is botched, however, leaving Hansel – now Hedwig – with a dysfunctional one-inch mound of flesh between her legs, the eponymous "Angry Inch".

Berke Landers (Ben Foster) and his girlfriend Allison (Melissa Sagemiller) were the quintessential high-school couple who grew up together and eventually fell in love, but she breaks up with him immediately after the film begins. This leads to an opening musical number of "Love Will Keep Us Together" by Vitamin C, imagined by Berke. He seeks advice from his embarrassing parents Frank (Ed Begley Jr.) and Beverly Landers (Swoosie Kurtz), who are hosts of a relationship advice show called Love Matters, but they don't help with the situation and constantly focus on his sex life and sexuality throughout the film. Allison then starts a relationship with Striker (Shane West), a 'foreign' student who was once the lead singer of a boy band called "The Swingtown Lads". When Allison and Striker audition for the school's upcoming musical, Berke desperately tries to win Allison back by also auditioning for the play, despite having no theatrical talent and having a busy schedule as a member of the basketball team. Meanwhile, Berke's friends Felix (Colin Hanks) and Dennis (Sisqó) try to find a new girlfriend for him.
