Synopsis
Steve Catlin was known as a real lady-killer. But it's his new bride's mysterious death that causes his former wife, Edie Ballew, to question how accurate that nickname really is. The plot involves Hemingford, Nebraska, a town near Gatlin (the original film's setting). While at the drive-in on Halloween during the annual showing of The Town That Dreaded Sundown, Jami Lerner (Addison Timlin) and Corey Holland (Spencer Treat Clark) are watching the film. Realizing that she is not enjoying it, they decide to leave. Parked in a secluded area, they begin to talk and kiss, but Jami sees the Phantom in the woods. They decide to leave but the Phantom breaks the window and makes them get out of the car while pointing a gun at them. He makes Holland remove his pants and lie on the ground. He tells Jami to turn around and to not look back. The Phantom begins to slash Corey to death. Jami runs off but is caught when she falls. He tells her, "This is for Mary. Make them remember." Jami walks back to the drive-in and collapses. The next morning, police interview her at the hospital. Later that night, Jami and her grandmother, Lillian (Veronica Cartwright), watch the news about the attack. Jami asks her grandmother what she remembers from the original attacks. She researches the crimes. The next day, she visits Corey's funeral.

The film revolves around a series of murders of young women. The opening scene features an unnamed blonde woman (Betty Boatner) running along a mountain road, wearing only her undergarments. She is clearly scared for her life. She enters a city park and manages to reach a telephone booth. Her attempt to escape her assailant ends in failure, when the man also reaches the booth. He quickly kills her and leaves her corpse next to a lake. The corpse is soon discovered by visitors of the park. The scene shifts to a police station, where Police Lieutenant Matt Carson (Kenne Duncan) receives a phone report on the latest murder. He instructs subordinate Officer Kline (Fred Mason) to gather information from the witnesses who called in the discovery of the body. Then he summons his associate Sergeant Randy Stone (Duke Moore) and they head out towards the crime scene. They take note of the youthful appearance of the victim and suspect a connection to the "smut picture racket".

In a suburban Miami house, a young woman arrives home and turns on her portable radio which broadcasts news of the latest of several recent murders. The woman turns off the radio and begins to take a bath. Suddenly a gray-haired, wild-eyed man appears in the bathroom and brutally stabs the woman in her left eye, killing her. Isabella (Francesca Ungaro), one of many beautiful models employed at a fashion house, is walking through the grounds that lead to the establishment one night when she is attacked and violently killed by an assailant wearing a white featureless mask. Police Inspector Sylvester (Thomas Reiner) is assigned to investigate the murder and he interviews Max Marian (Mitchell), the manager who co-manages the salon with his lover, the recently widowed Countess Cristina Como (Bartok). Max attests that he can not provide any information whatsoever that can assist the inspector, but as the investigation continues all of the fashion house's various sins, including corruption, abortions, blackmail and drug addiction, begin to come to light. It is revealed that Isabella had kept a diary detailing these vices, and suddenly almost every employee becomes nervous.
