Business tycoon and billionaire Damian Hale (Ben Kingsley) is master of his own universe, until he is diagnosed with terminal cancer. Now on his deathbed, he finds a business card directing him to a man named Professor Albright (Matthew Goode), who informs him about a radical medical procedure called "shedding", in which one's consciousness is transferred to an artificially grown healthy body. Damian decides to undergo the procedure and engineers his own public death. In 1954, two U.S. Marshals - Edward "Teddy" Daniels and his new partner, Chuck Aule - travel to the Ashecliffe Hospital for the criminally insane on Shutter Island located in Boston Harbor as part of an investigation into the disappearance of patient Rachel Solando, incarcerated for drowning her three children. The sole clue is a note left by Solando, which reads: "The law of 4; who is 67?" Shortly after arrival, a storm prevents their return to the mainland for several days. Daniels finds the staff confrontational: the lead psychiatrist, Dr. John Cawley, refuses to hand over records of the hospital staff; Solando's doctor, Dr. Lester Sheehan, had left on vacation after her disappearance, and they are barred from searching Ward C and told that the lighthouse on the island has already been searched, so there is no need to search it. When a patient is being interrogated by Daniels, with a subterfuge she sends Aule away for a few seconds and scribbles "RUN" in Daniels' notepad.

A man is seated in a diner who is revealed as Harold, a traveling salesman. He is joined by a gentleman who makes him uncomfortable. After leaving the diner, Harold is found dead, which leads to FBI Agents Thomas Mackelway (Aaron Eckhart) and Fran Kulok (Carrie-Anne Moss) being brought in on the case. There appears to be history between Mackelway and Kulok and a cloud of competency is addressed by flashbacks of Mackelway's recent suspension and issues he's undergone. Frank Falenczyk (Ben Kingsley) is a hit man for his Polish mob family in Buffalo, New York. In Kraków during World War II, the Germans had forced local Polish Jews into the overcrowded Kraków Ghetto. Oskar Schindler, an ethnic German, arrives in the city hoping to make his fortune. While Suraj trains Naina they develop feelings for each other. After an argument, Suraj refuses to meet Naina and she decides to go after Gokul all by herself. One day Gokul kidnaps Naina's younger sister Dia from school in order to scare and warn Naina with what risk she is playing. That day Naina's mother decides to leave for Naintal immediately as her daughter's life is at risk. However Naina cannot control her hatred, and wanted to avenge her sister at any cost. Naina lays a trap for Gokul and tries to kill him but Gokul ties her up and tries to rape her like her sister. Suraj arrives at her house and fights Gokul but ends up being stabbed. Naina gets free and manages to shoot Gokul dead.
