It was written and directed by the Cuban filmmaker, Leon Ichaso. It premiered at the Montreal Film Festival on 31 August 2001. It then received a limited theatrical release in the United States on 13 December 2001.

Miklo is a man of Mexican and Caucasian ethnicity who grew up in El Pico Aliso barrio in east Los Angeles. Through the course of the film, several flashbacks are shown involving the Skuggs brothers, including (in the beginning of the film) the drug-overdose death of their mother Ella (Khandi Alexander), the non-fatal shooting of their drug-addicted musician father, Arthur Romello "A.R." Skuggs (Clarence Williams III) (ultimately at the hands of the man they would later work for—Gus Molino (Abe Vigoda), and a scene where Roemello is offered a full scholarship to Georgetown. Roemello, as a teenager (Dulé Hill) avenges his father’s shooting by shooting and killing Sal Marconi (Raymond Serra), a business partner of Gus’s. After contemplating for a while, Roemello decides to quit dealing and start a new life with his girlfriend, Melissa (Theresa Randle), to the disdain of Raynathan, who is scared and hesitant to leave the drug game. However, Roemello learns that getting out is nowhere near as easy as getting in.

In late 19th century Cartagena, a river port in Colombia, Florentino Ariza falls in love at first sight with Fermina Daza. They secretly correspond and she eventually agrees to marry but her father discovers their relationship and sends her to distant relatives (mainly her grandmother and niece). When she returns some years later, Fermina agrees to marry Dr. Juvenal Urbino, her father's choice. Their fifty-year marriage is outwardly loving, but inwardly marred by darker emotions. Fermina's marriage devastates Florentino, who vows to remain a virgin, but his self-denial is thwarted by a tryst. To help him get over Fermina, his mother throws a willing widow into his bed and he discovers that sex is a very good pain reliever with which he replaces the opium that he had habitually smoked. He begins to record and describe each of his sexual encounters, beginning with the widow, and eventually compiling over 600 entries. A lowly clerk, he plods resolutely over many years to approach the wealth and social standing of Dr. Urbino. When the now-elderly doctor dies suddenly, Florentino immediately and impertinently renews courting Fermina.

In 1961, the financially strapped author Pamela "P. L." Travers reluctantly travels from her home in London to Los Angeles to work with Walt Disney at the urging of her agent, Diarmuid Russell.
