In 2154, Earth is overpopulated and polluted. Most of the earth's citizens live in poverty, on the edge of starvation, and with little technology and medical care. The rich and powerful live on Elysium — a gigantic, terraformed space habitat located in Earth's orbit. Elysium is technologically advanced; such technology includes Med-Bays: medical machines that can cure all diseases, reverse the aging process and regenerate new body parts. Returning home with his mother Vinn (Talisa Soto) from a trip to Berlin, Michael Gant, the son of Defense Intelligence Agency (DIA) director Robert Gant (Gregg Henry), is kidnapped and his security detail is wiped out by the attacker, ex-DIA agent Sever (Lucy Liu). FBI agent Jeremiah Ecks (Antonio Banderas), left the agency after his wife Rayne was killed in a car bombing. His old boss, Julio Martin (Miguel Sandoval), asks him to investigate the Gant case. He claims that Rayne is still alive and he will give Ecks the information on her whereabouts, if he helps take down Sever. Ecks agrees and realizes Sever must be an orphan Chinese girl the DIA adopted to train as a covert operative and assassin with "no fear, no conscience and no morality." Meanwhile, Gant executes the only survivor of Michael's security detail. He then orders his elite agents, led by A. J. Ross (Ray Park), to pursue Sever and rescue Michael.

The story centers around a group of people that arrive at Hart Island, which has recently been purchased by real estate tycoon Rupert King to build what he calls Hope City, supposedly to help the poor and homeless of the city. The evil emperor Shao Kahn, along with his generals, Motaro, Sheeva, Rain, and Ermac, opens a portal from Outworld to Earthrealm and has reclaimed his queen Sindel, who is Kitana's long-dead mother. Earthrealm is therefore in danger of being absorbed into Outworld within six days, a fate which Liu Kang and the others must fight to prevent. Kahn fights and quickly kills Johnny Cage during the confrontation by snapping his neck, and the remaining Earthrealm warriors must regroup and think of an idea to defeat Shao Kahn. An emotionally guilt-ridden Sonya Blade enlists the help of her old partner, Jax, while Kitana and Liu Kang search for a Native American shaman named Nightwolf, who seemingly knows the key to defeating Kahn. On the way, they run afoul of the cyborg Smoke. Kitana and Liu Kang dispatch him with the aid of Sub-Zero, but Scorpion suddenly appears, attacks Sub-Zero, and kidnaps Kitana.
