The police investigate a string of murders committed by the Creeper (Rondo Hatton), a mysterious killer with a hideously disfigured face. The Creeper attacks and murders Professor Cushman (John Hamilton), a professor from the nearby Hampton University. Later that night, the killer approaches a woman named Joan Bemis (Janelle Johnson) in front of her home and identifies himself as Hal Moffet. Joan screams hysterically at the sight of him until he is driven to kill her. When police cars approach, the Creeper climbs the fire escape of a city tenement building to escape and enters the apartment of Helen Paige (Jane Adams), a blind pianist. Unable to see the Creeper's deformed face, Helen is not afraid of the intruder, even when he admits he is fleeing from the police. Struggling sculptor, Marcel De Lange (Martin Kosleck) is depressed about the events going on in his life, and decides to commit suicide. But just as he's about to kill himself, he spots and saves a madman, named "The Creeper" (Rondo Hatton) from drowning. Shortly afterward, he takes the disfigured man into his care. Marcel also makes the Creeper the subject of his next sculpture and calls it his best creation. The film begins with the sentencing of Jean-Baptiste Grenouille (Ben Whishaw), a notorious murderer. Between the reading of the sentence and the execution, the story of his life is told in flashback, beginning with his abandonment at birth in a French fish market. Raised in an orphanage, Grenouille grows into a strangely detached boy with a superhuman sense of smell. After growing to maturity as a tanner's apprentice, he makes his first delivery to Paris, where he revels in the new odors. He focuses on a redheaded girl (Karoline Herfurth) selling yellow plums, following her and repeatedly attempting to sniff her, but startles her with his behavior. To prevent her from crying out, he covers the girl's mouth and unintentionally suffocates her. After realizing that she is dead, he strips her body naked and smells her all over, becoming distraught when her scent fades. Convicted murderer Seth Baxter wakes up chained to a table beneath a pendulum blade. A videotape informs him that he can release himself by crushing his hands between two presses. He does so, but the blade still swings down and cuts him in half as someone watches through a hole in the wall. In the meatpacking plant, FBI Agent Peter Strahm guns down Jeff Denlon and is locked in the sickroom. He discovers a hidden exit with a microcassette recorder, which urges him to stay in the sickroom. He ignores it and is attacked in the tunnel by a pig-masked figure, waking up with his head sealed in a box which quickly fills with water; he survives by performing a tracheotomy using his pen. Peter Neal, an American writer of violent horror novels, is in Italy to promote his latest work, Tenebrae. He is accompanied by his literary agent, Bullmer and his assistant, Anne. Neal is unaware that he has also been followed to Rome by his embittered ex-wife, Jane. Just before Neal arrived in Rome, Elsa, a young female shoplifter, was murdered with a razor by an unseen assailant. The murderer sends Neal a letter informing him that his books have inspired him to go on a killing spree.
