In September 1942, Captain Yamagami (Susumu Kurobe) is ordered to rendezvous the Yamato with the Combined Fleet that is gathering at the Truk Islands (aka Chuuk Islands) in Micronesia, a key strategic point in the South Pacific. With beautiful clear blue skies above and surrounded by coral reefs below, this South Seas paradise became a strong base for the Combined Fleet and the front lines of the naval war; a place where many fierce battles were fought. Decades later, the sea bed surrounding the Truk Islands is still littered with the remains of more than 60 warships and airplanes.

Sachiko Hanai (Emi Kuroda) works as a call girl or soap girl specializing in sexual roleplay, or cosplay. In her work, Sachiko portrays a home-tutor, hence the film's original title, Horny Home Tutor: Teacher’s Love Juice. While at a café after one job, she witnesses an altercation between two men, one a North Korean and the other from the Middle East, who turn out to be spies in the middle of a transaction. When the argument escalates to gun-play, Sachiko foolishly starts to take a cell-phone picture of the incident and is shot in the forehead. Rather than killing her, the bullet lodges in her brain and gives her extraordinary mental powers, including the ability to understand languages of which she previously had no knowledge, arcane philosophical insight, advanced mathematical knowledge, and ESP. After fleeing from the scene, she finds a metal cylinder in her pocket which contains a cloned copy of the finger of United States President George W.

Confronting a group of tin knights called Swordmen of the Wind (風の剣士, Kaze no Kenshi) escorting a picture frame, Kouga obtains a sword when they are attacked by a formless monster, a Nanashi (ナナシ). Once the two surviving knights run off, Kouga sees a blue skinned girl emerging from the picture frame. The girl introduces herself as Meru and that she is like every resident in the Promised Land: A living object. Zeiram (Mizuho Yoshida) is an alien of unknown origin. It appears to be a very large, imposing humanoid wearing a wide, circular hat and a ragged cloak, and its appearance is usually accompanied (at least on the film's soundtrack) by an eerie chant. However, its true form is actually the small Noh-like face on its hat. This face is its only weak point; the rest of the body is almost indestructible and regenerates quickly as long as Zeiram is able to feed, which it does by extending the little face on the end of a fleshy tentacle to take bites out of people. By doing this, it also absorbs the genetic material of its victims, which it uses to generate smaller, weaker monsters to do its bidding. These monsters seem to be partial clones of whatever creature's genetic material was used in their creation (in Zeiram 2, a dog is killed by Zeiram and the resulting monster looks vaguely canine). Zeiram itself is able to change its own form to a large degree, and appears in many different forms throughout the series. It can produce many different weapons such as guns and swords from its body, although these are probably weapons taken from its victims and stored inside itself. In addition to consuming organic tissue through its core, if it sustains enough damage, Zeiram is able to ingest a victim's entire body, keeping that person alive for some time inside itself while it digests them, regenerating itself and drawing on their genetic material and even their memories to create more advanced clones.

After miraculously recovering from a bullet wound to the head, Gulf War veteran Jack Starks (Adrien Brody) returns to Vermont in 1992, suffering from periods of amnesia. While walking, he sees a young girl, Jackie (Laura Marano), and her alcoholic mother (Kelly Lynch) in despair beside their broken-down truck. Starks and Jackie quickly form a certain affinity; she asks him to give her his dogtags and he does so. He gets the truck started for them and continues on his way. Shortly after, a man driving along the same highway gives Jack a ride and they get pulled over by a policeman. The scene changes: Starks is found lying on the deserted roadside near the dead policeman, with a slug from the policeman's gun in his body. The murder weapon is on the ground nearby. Although he testifies there was someone else at the scene, he is not believed because of his amnesia. Starks is found not guilty by reason of insanity and is incarcerated in a mental institution.
