Set, like the previous films, in the fictional port town of Blue Bay, Marie Clifton (Laine) is set to inherit two beautiful diamonds, called the "mother and daughter". Marie's step-father, Jay Clifton (Johnson), challenges the will claiming that he believes Marie is not ready for the responsibility, but actually wishing to claim the diamonds for himself. At a sexual education seminar, physician Chad and probation officer Kristen Richards (Meyer) discuss sex crimes. Richards reveals that she was a victim of an anonymous rapist many years before.

Julia lives in a small town in Washington called Cold Rock. She is the town's local nurse, widowed by her husband, who was the town's doctor. Cold Rock was formerly a prosperous mining town, but has become poverty-stricken. There is little work, the school has been closed and the town is virtually vanished from the map. During her day, Julia visits several families. Jenny and Carol live with their mother and her violent, alcoholic boyfriend, who has impregnated Carol. Despite this, their mother did not kick him out, drawing Julia's ire. Instead, she sends away Carol and her baby, whom Julia helped to deliver. Because of her rough life, Jenny is selectively mute, communicating through a journal she carries. She also sketches things she's seen, including "the tall man". This is a local legend or urban myth surrounding a being who abducts local children who are never seen again.

Jess (Gellar) is a sweet-natured but driven lawyer who puts her career ahead of her personal life and ahead of her marriage to her artist husband, Ryan (Landes). The couple is on the verge of their first wedding anniversary, and though they are happy, the thorn in the side of their relationship is Ryan's brother Roman (Pace). In 2021, Johnny is a "mnemonic courier" with a data storage device implanted in his brain, allowing him to discreetly carry information too sensitive to transfer across the Net, the virtual-reality equivalent of the Internet.
