Hanna and Simon are in a long-term relationship which, though loving, has grown sexually unexciting. Soon after Simon's mother dies from advanced pancreatic cancer, he discovers that he has testicular cancer and must undergo surgery and chemotherapy. The night of his surgery, Hanna has a sexual encounter with a man called Adam, and Simon learns that he had fathered a child seventeen years earlier, although the woman opted for an abortion. Simon, who had assumed that either he or Hanna was infertile, is assured that he should still be able to have children after his surgery. During his recovery, Simon and Hanna decide to finally wed. Shortly beforehand, Simon encounters Adam at the pool and begins an affair with him as well.

In 1944 in the Republic of Salò, the Fascist-occupied portion of Italy, four wealthy men of power, the Duke, the Bishop, the Magistrate, and the President, agree to marry each other's daughters as the first step in a debauched ritual. They recruit four teenage boys to act as guards and four young soldiers (called "studs", "cockmongers", or "fuckers"), who are chosen because of their big penises. They then kidnap nine young men and nine young women and take them to a palace near Salò. Accompanying them are four middle-aged prostitutes, also collaborators, who recount arousing stories for the men, who sadistically exploit their victims.

À la mort de Ned, le père de Jonathan, le trio se retrouve pour l'enterrement à Phoenix et Claire annonce qu'elle est enceinte. Sur le chemin du retour, elle achète une maison perdue dans la nature à Woodstock où ils s'installent tous les trois. Tandis que Claire élève sa fille Rebecca, Bobby et Jonathan ouvrent un café. Jonathan est atteint de la maladie de Kaposi, Claire vit mal l'intimité de Bobby et Jonathan. Finalement elle quitte la maison au bout du monde avec sa fille pour visiter sa mère à Philadelphie.
