The film is about a woman who experiences frightening visions after visiting an insane asylum where one of the inmates claims to be Count Dracula (here following the Hungarian spelling Drakula). She has trouble determining whether the inmate's visions are real or merely nightmares.

The film opens with Michael and Madeline attempt to conceive. It is soon apparent that they have succeeded. Michael's parents arrive for dinner: a vegan meal prepared by a now visibly pregnant Madeline. Michael's domineering mother Vivian grimaces at the food and suggests - in a passive aggressive fashion - that a more conventional diet would be healthier for the unborn baby. The conversation turns to the couple's unsuccessful search for a suitable obstetrician. Madeline has rejected Vivian's preferred doctor and friend, Dr. Sohn. Instead she has decided to consult Patricia, her old friend and midwife at an independent clinic. Vivian does not hide her disapproval. Though Michael is also skeptical, the couple visit the clinic for a tour. They meet with Patricia, who describes her natural approach to birthing. Madeline reveals that she and Michael have conceived twice before, but both pregnancies were unsuccessful.
Vampires are finding their own undead bodies being mutated by the pollution of the host's blood, tainted with hard drugs and sexually transmitted diseases, alcohol, diabetes, anti-depressants, and cigarettes: substances that change the blood and makes it undrinkable for vampires. The scarcity of good blood has incited an underground civil war between various groups of vampires. A clique of 4 vampires, led by Benedict (Mark Hengst), struggle to find sustenance by seeking victims with untainted blood. The group is being stalked by a samurai sword-wielding vampire-killing Priest (Tim Thomerson), who leaves a note on the bodies of slain vampires in the form of a playing card inscribed with the words "Live Evil". In order to survive both their race's own fierce infighting and the biological pollution found in human blood, the group desperately seeks out Max (Ken Foree), a "blood pusher" who steals from hospital blood banks to offer the freshest and purest blood around. But they may not have time to enjoy it, because the vengeful Priest is hot on their trail.
The film focuses on Second Lieutenant Bart Gregory (David Anders) who has been killed under mysterious circumstances in Iraq. After his friends and girlfriend Janet (Louise Griffiths) attend his funeral, Bart awakens in his grave. Enlisting the help of his best friend, Joey Leubner (Chris Wylde), Bart begins to understand and learn how to deal with his new undead state; mainly, the fact that Bart needs blood to hold back decomposition and that he returns to a state of in-animation during daylight hours. Joey does research online to find out what Bart is and seems to be stuck between a Zombie and Vampire, finally stating that Bart is a Revenant.
