The film opens showing Louis "Louie" Zamperini flying as a bombardier of a United States Army Air Forces B-24 Liberator bomber, during an April 1943 bombing mission against the Japanese-held island of Nauru. The plane is badly damaged in combat, with a number of the crew injured. The hydraulics of the plane are shot and damaged, but the pilot, Phil, manages to bring it to a stop at the end of the runway thanks to a flat tire.

On a brief trip back to London, earnest, bookish bacteriologist Walter Fane (Edward Norton) is dazzled by Kitty Garstin (Naomi Watts), a vivacious and vain London socialite. He proposes; she accepts ("only to get as far away from [her] mother as possible"), and the couple honeymoon in Venice. They travel on to Walter's medical post in Shanghai, where he is stationed in a government lab studying infectious diseases. Six months after the events of the first film, Neo and Trinity are now lovers. Morpheus receives a message from Captain Niobe of the Logos calling an emergency meeting of all of Zion's ships. Zion has confirmed the last transmission of the Osiris: an army of Sentinels is tunneling towards Zion and will reach it within 72 hours. Commander Lock orders all ships to return to Zion to prepare for the onslaught, but Morpheus asks one ship to remain in order to contact the Oracle. The Caduceus receives a message from the Oracle, and the Nebuchadnezzar ventures out so Neo can contact her. One of the Caduceus crew, Bane, encounters Agent Smith, who takes over Bane's avatar. Smith then uses this avatar to leave the Matrix, gaining control of Bane's real body.

Neo and Bane lie unconscious in the medical bay of the ship Hammer. Meanwhile, Neo finds his digital self trapped in a virtual subway station—-named, "Mobil Ave.", "mobil", being an anagram for "limbo"-a transition zone between the Matrix and the Machine City. In that subway station, he meets a "family" of programs, including a girl named Sati, whose father tells Neo the subway is controlled by the Trainman, an exiled program loyal to the Merovingian. When Neo tries to board a train with the family, the Trainman refuses and overpowers him.

In 2010, United Nations negotiator Edward Douglas (David Thewlis) survives a plane crash in the Java Sea and is eventually rescued by a passing boat. Aboard, a man called Montgomery (Val Kilmer) tends to him, and after telling him the boat has no radio, he promises Douglas the captain will take him to Timor after dropping him off. However, when they arrive at Montgomery's destination referred to as "Moreau's Island", he instead advises Douglas to stay with him, ostensibly so he can use the radio on the island.

Frank Castle is the city's most wanted, and most mysterious, vigilante, known as "The Punisher". He has killed 125 criminals in the past five years. An ex-police officer, Castle's family was murdered in a mob hit, in which Castle was also thought to have perished. This combined with Castle's furtive methods of attack have made the Punisher's identity a mystery to the general public. Living in the sewers and waging a one-man war against organized crime, his only friend is an old alcoholic named Shake, a former stage actor who speaks in rhyme for reasons unexplained. Now legally declared dead, Castle strikes back from beyond the grave, killing mobsters wherever he can find them. Due to his war against them, the Mafia families have weakened, forcing one of the family leaders, Gianni Franco, to come out of retirement. Franco has a plan to unify the families. This, however, has attracted the attention of the Yakuza, Asia's most powerful crime syndicate. Led by Lady Tanaka, the Yakuza decide to take over the Mafia families and all of their interests. In order to sway the mobsters to their cause, they kidnap their children and hold them for ransom.
