Dans les années 1870 à New York, l'entrepreneur américain Phineas Taylor Barnum (Hugh Jackman) cherche à développer une activité de divertissement pour améliorer les conditions de vie de sa famille. C'est dans ce contexte qu'il crée le freak show, un spectacle composé de comédiens aux caractéristiques et capacités uniques (femme à barbe, nain, trapézistes, …), et invente le premier "cirque", le cirque Barnum. Ces spectacles sont globalement appréciés, mais également très critiqués par la presse de la haute société américaine. Pour améliorer son image, M. Barnum s'associe avec Philip Carlyle (Zac Efron), un jeune dramaturge populaire et influent qui va lui permettre de se faire connaître au-delà même des frontières américaines. Lors d'une visite au palais royal d'Angleterre, M. Barnum fait la connaissance de Jenny Lind (Rebecca Ferguson), une chanteuse à succès sur le continent européen, qu'il convainc de venir chanter aux États-Unis sous son aile. Grâce au talent de Miss Lind, ce nouveau spectacle est un succès et M. The film opens in October 1944, in the Auschwitz-Birkenau extermination camp. A small group of Sonderkommandos, prisoners assigned to dispose of the bodies of other dead prisoners, are plotting an insurrection that, they hope, will destroy at least one of the camp's four crematoria and gas chambers. They are receiving firearms from Polish citizens in the nearby village and gunpowder from the UNIO munitions factory; the female prisoners who work in the UNIO are smuggling the powder to the men’s camp amid the bodies of their dead workers. When the women's activity is eventually discovered by the Germans they are savagely tortured, but they don't reveal the plot.

During the summer in Brighton Beach, Brooklyn, elderly widow Sara Goldfarb (Ellen Burstyn) constantly watches television, particularly infomercials hosted by Tappy Tibbons (Christopher McDonald). After receiving an unexpected phone call that she has won a spot to participate on a television game show, she becomes obsessed with regaining the youthful appearance she possesses in an old photograph from her son Harry's (Jared Leto) graduation many years earlier. In order to fit into her old red dress seen in the picture, the favorite one of her deceased husband Seymour, she goes on a crash diet. In order to reach her goal sooner, she goes to a doctor to discuss weight loss. The doctor gives her a prescription for weight-loss amphetamine pills throughout the day and a sedative at night. Harry warns her about amphetamine dependence and risk of life-threatening consequences, but she rebuffs him and insists that the chance to be on television has given her a reason to live. As the months go by, Sara's tolerance for the pills adjust and as a result she is no longer able to feel the same high the pills once gave her. When her invitation has still not arrived, she wrongfully increases her dosage from double to triple and, as a result, begins to suffer from amphetamine psychosis. Soon, her delusions worsen and she is driven to the brink of madness when she suffers a hallucination that she appears on the game show as the principal subject while being attacked by her monstrous, anthropomorphized refrigerator.

The film begins with a long tracking shot focusing on a destitute young woman named Olive Stanton (Emily Watson). She is sleeping illegally in a theater, being awakened and kicked out. The shot continues as she slowly walks down the street following the sound of the song Nickel Under My Foot. When she arrives at the source of the music, the camera pans up the side of the building and moves inside where we are introduced to the playwright Marc Blitzstein (Hank Azaria), who is attempting to write the songs and put together the musical The Cradle Will Rock. Acting as Blitzstein’s conscience/mentors are a vision of his deceased wife and, later, an imaginary Bertolt Brecht. Brecht was a radical playwright who stressed the importance of breaking down the wall between the audience and actors.
