Richard P. Rubinstein (born June 15, 1947 in Brooklyn, New York) is an American film and television producer, who has worked mainly in the science fiction and horror genres. In the 1970s and 1980s he collaborated frequently with horror director George A. Romero, including on the seminal 1978 zombie film Dawn of the Dead and the 1984-1988 anthology horror television series Tales from the Darkside. In the 1980s and 1990s Rubinstein produced a substantial number of projects based on the writings of horror novelist Stephen King.

A young boy named Billy gets yelled at and slapped by his father, Stan, for reading a horror comic titled Creepshow. Stan reminds his wife that he had to be hard on Billy because he does not want their son to be reading such "crap". As Billy sits upstairs cursing his father with hopes of him rotting in Hell, he hears a sound at the window, which turns out to be a ghostly apparition in the form of The Creep from the comic book, beckoning him to come closer.

Billy (Ed Harris) leads a traveling troupe that jousts on motorcycles. "King William", as he styles himself, tries to lead the troupe according to his Arthurian ideals. However, the constant pressure of balancing those ideals against the modern day realities and financial pressures of running the organization are beginning to strain the group. Billy is also plagued by a recurring dream of a black bird. Tensions are exacerbated by Billy's constantly pushing himself despite being injured and the arrival of a promoter named Bontempi (Martin Ferrero), who wants to represent the troupe.

After finishing a long shift, nurse Ana returns to her suburban neighborhood and her husband, Luis. Caught up in a scheduled date night, they miss an emergency news bulletin. The next morning, a neighborhood girl enters their bedroom and kills Luis, who immediately reanimates as a zombie and attacks Ana. She flees in her car but crashes and passes out. Upon waking, Ana joins with Police Sergeant Kenneth Hall, electronics salesman Michael, petty criminal Andre and his pregnant wife, Luda. The story follows a reporter named Richard Dees as he follows, attempting to catch up with, a murderer who kills his victims in a vampiristic style. At a government laboratory in rural California, a weaponized version of influenza (called Project Blue) is accidentally released, immediately wiping out everyone on staff except for military policeman Charles Campion and his family, who flee the base. However, Campion is already infected by the superflu, nicknamed "Captain Trips", and spreads it to the outside world. That evening, Campion crashes his car at a gas station in East Texas where Stu Redman (Gary Sinise) and some friends have gathered. When they investigate, they find Campion dying of the flu next to his wife and baby daughter, who are already dead. Campion tells Stu with his dying breath that he was followed from the base by a mysterious figure, and states "You can't outrun the Dark Man". The next day, the U.S. military arrive to quarantine the town. While the other townspeople quickly become ill and die, Stu remains healthy and is confined at a CDC facility in Vermont order to study a possible cure. This proves futile and the superflu rages unchecked, causing civilization to collapse and killing over 99% of the population of the entire world in less than two months.

A delivery truck pulls up to a newsstand in a small town where a young boy named Billy (named after and confused with the boy from Creepshow) arrives eagerly waiting for it. The truck's back shutter opens to reveal a sinister figure who drops off a package onto the sidewalk — the latest issue of Creepshow, much to Billy's delight. As the film momentarily turns into an animation, the package opens of its own accord, (revealing the cover of the comic is the same as the cover in the final scene of the previous film).
