He is particularly noted for his appearances in 21 of Akira Kurosawa's 30 films, including as a lead actor in Drunken Angel (1948), Ikiru (1952) and Seven Samurai (1954).

In 1835, Gentetsu Nishiyama begins preaching the prophecies of Michel de Nostradame using a copy of his book, "Centuries." When Nishiyama gets persecuted by the Tokugawa Shogunate for supposed heresy, his wife and son flees with the book in hand, passing down the knowledge to future generations. At the onset of World War II, his descendant, Gengaku, is interrogated by an Imperial Japanese Army officer about the family's continued preaching of the prophecies, which predicted the rise of Nazism and the Axis defeat.

Au hasard de ses pérégrinations, Zatoïchi se retrouve à passer près de Kasama, son village natal qu'il a quitté depuis vingt ans. Torajiro Kuruma (Kiyoshi Atsumi), returns for the first time in 20 years to the neighborhood where he was born, Shibamata in Katsushika, Tokyo, He comes back in the middle of Taishakuten temple's Koshin Shinko Festival. After jumping into the festival procession, he has a teary reunion with his uncle (Shin Morikawa), aunt (Chieko Misaki), and his sister Sakura (Chieko Baisho). The next day, he joins his sister at her at a matchmaking meeting with the son of a company president. Tora's boorish and drunken behavior at the meeting ruins Sakura's chances of marriage. After getting into a huge fight with his uncle over his behavior at the meeting, he leaves home and heads to Nara. While there, he meets and falls in love with a young woman named Fuyuko. They return to Shibamata together and spend time together as if they were couple. At the same time, Sakura meets a print shop worker, Hiroshi (Gin Maeda). While Sakura and Hiroshi get married, Tora's relationship with Fuyuko is fleeting, as she is engaged to another man. Broken-hearted, Torajiro leaves after writing a letter of advice for Fuyuko, asking her to help take care of his sister and her new baby.

The film takes place in Koishikawa, a district of Edo (the former name of the city of Tokyo), in the 19th century. Young Dr. Noboru Yasumoto (Yūzō Kayama) is the film's protagonist. Trained in a Dutch medical school in Nagasaki, the arrogant Yasumoto aspires to the status of personal physician of the Shogunate, a position currently held by a close relative; his father is already a well-established, highly competent physician. Yasumoto believes that he should progress through the safe, and well-protected, army structure of medical education. However, for Yasumoto's post-graduate medical training, he has been assigned to a rural clinic under the guidance of Akahige ("Red Beard"), Dr. Kyojō Niide (played by Toshiro Mifune). Dr. Niide may seem like a tyrannical task master, but in reality he is a compassionate clinic director. Initially, Yasumoto is livid at his posting, believing that he has little to gain from working under Akahige. Dr. Yasumoto feels that Dr. Niide is only interested in his medical notes and soon rebels against the clinic director. He refuses to wear his uniform, disdains the food and spartan environment, and enters the forbidden garden where he meets "The Mantis" (Kyōko Kagawa), a mysterious patient that only Dr. Niide can treat.
