The story may not be linear and exhibits several instances of temporal disruption. A dark-haired woman (Harring) escapes her own murder, surviving a car accident on Mulholland Drive. Injured and in shock, she descends into Los Angeles and sneaks into an apartment which an older, red-headed woman has just vacated. An aspiring actress named Betty Elms (Watts) arrives at the same apartment and finds the dark-haired woman confused, not knowing her own name. The dark-haired woman assumes the name "Rita" after seeing a poster for the film Gilda (1946), starring Rita Hayworth. To help Rita remember her identity, Betty looks in Rita's purse, where she finds a large amount of money and an unusual blue key.

An earthquake strikes the city of Los Angeles. Mike Roark (Tommy Lee Jones), the head of the city's Office of Emergency Management, insists on coming to work to help out with the crisis, even though he has taken a vacation with his daughter Kelly (Gaby Hoffmann). His associate Emmit Reese (Don Cheadle) notes the quake caused no major damage, but seven utility workers are later burned to death in a storm drain, one escaped and survived but was severely burned on one side of his face at MacArthur Park. As a precaution, Roark tries to halt the subway lines which run parallel to where the deaths took place, but Los Angeles MTA Chairman Stan Olber (John Carroll Lynch) declines, feeling there is no threat to the trains. Against regulations, Roark and a colleague Gator Harris (Michael Rispoli) venture down the storm sewer in the park to investigate. They are nearly burned to death when hot gases suddenly spew out of a crack in the concrete lining and flood the tunnel. Geologist Dr. Amy Barnes (Anne Heche) believes a volcano may be rapidly forming beneath the city with magma flowing underground (similar to the formation of the Mexican volcano Parícutin which emerged and grew tremendously in just one week) but cannot come up with enough evidence for Roark to take action.

In the Mojave Desert in 1982, a man named Nix (von Bargen) has gathered a cult in an isolated house. Nix calls himself "The Puritan" and has the ability to use real magic. He plans to sacrifice a young girl that he has kidnapped. While Nix is preaching to his followers, a group of former cult members –Swann (O'Connor), Pimm (Traylor), Quaid (Latimore) and Jennifer Desiderio (Tousey)– arrive to stop him. After the initial confrontation with the cultists, Nix's assistant Butterfield escapes and Swann is attacked by Nix. Nix uses his powers, inducing in Swann the vision of “Flesh with a God's Eyes", causing him to see his friends as monstrous figures. The kidnapped girl shoots Nix through the heart with Swann's gun. Quaid and Jennifer shoot him until he falls. Swann snaps out of his vision and fastens an ironwork mask over Nix's head, who appears to die. Swann declares that they will bury Nix so deep that no one will ever find him.

The film has several plot lines, one each for Ralphie, his father and his mother, followed by one involving Ralphie and his dad on a fishing trip. In 1863, First Lieutenant John J. Dunbar is wounded in the American Civil War. Choosing suicide over having his foot amputated, he takes a horse and rides up to and along the Confederate front lines, distracting them in the process. Despite numerous pot shots, the Confederates fail to shoot him off his mount. Whilst they are distracted, the Union army attacks the line, and the battle ends in a Confederate rout. Dunbar survives, and is allowed to recover properly, receives a citation for bravery, and is awarded Cisco, the horse who carried him, as well as his choice of posting. Dunbar requests a transfer to the western frontier so he can see it before it disappears. Dunbar is initially posted at Ft. Hays, a large fort presided over by a mentally ill and suicidal major who despises Dunbar's enthusiasm. Dunbar requests that the major transfer him to a real frontier fort. Just before he takes his life, The major sends him to an abandoned fort, Fort Sedgewick, with Timmons, a mule wagon provisioner. Dunbar finds the fort deserted and in need of repair. Despite the threat of nearby Indian tribes, he elects to stay and man the post himself. He begins rebuilding and restocking the fort and prefers the solitude afforded him, recording many of his observations in his diary.
