The film takes place in Koishikawa, a district of Edo (the former name of the city of Tokyo), in the 19th century. Young Dr. Noboru Yasumoto (Yūzō Kayama) is the film's protagonist. Trained in a Dutch medical school in Nagasaki, the arrogant Yasumoto aspires to the status of personal physician of the Shogunate, a position currently held by a close relative; his father is already a well-established, highly competent physician. Yasumoto believes that he should progress through the safe, and well-protected, army structure of medical education. However, for Yasumoto's post-graduate medical training, he has been assigned to a rural clinic under the guidance of Akahige ("Red Beard"), Dr. Kyojō Niide (played by Toshiro Mifune). Dr. Niide may seem like a tyrannical task master, but in reality he is a compassionate clinic director. Initially, Yasumoto is livid at his posting, believing that he has little to gain from working under Akahige. Dr. Yasumoto feels that Dr. Niide is only interested in his medical notes and soon rebels against the clinic director. He refuses to wear his uniform, disdains the food and spartan environment, and enters the forbidden garden where he meets "The Mantis" (Kyōko Kagawa), a mysterious patient that only Dr. Niide can treat.

A ship carrying plutonium collides with a floating atoll off the eastern coast of the Philippines, one of many incidents occurring throughout the area. As the anomalous formation approaches Japan, a team of scientists led by Naoya Kusanagi (Akira Onodera) discover orihalcum amulets and a stone slab covered in Etrurian runes on the atoll. During the investigation, the atoll suddenly quakes, destroying the slab and throwing the scientists into the ocean. A year has passed since the battle between Gamera and the Gyaos, and Japan has struggled to rebuild its cities in the meantime. The military has kept a cautious vigil on the nation's coast, but so far Gamera has yet to return. Suddenly, a series of bizarre incidents reveal a new threat to the land of the rising sun. Sachiko Hanai (Emi Kuroda) works as a call girl or soap girl specializing in sexual roleplay, or cosplay. In her work, Sachiko portrays a home-tutor, hence the film's original title, Horny Home Tutor: Teacher’s Love Juice. While at a café after one job, she witnesses an altercation between two men, one a North Korean and the other from the Middle East, who turn out to be spies in the middle of a transaction. When the argument escalates to gun-play, Sachiko foolishly starts to take a cell-phone picture of the incident and is shot in the forehead. Rather than killing her, the bullet lodges in her brain and gives her extraordinary mental powers, including the ability to understand languages of which she previously had no knowledge, arcane philosophical insight, advanced mathematical knowledge, and ESP. Daigo Kobayashi (Masahiro Motoki) loses his job as a cellist when his orchestra is disbanded. He and his wife Mika (Ryōko Hirosue) move from Tokyo to his hometown in Yamagata, where they live in his childhood home that was left to him when his mother died two years earlier. It is fronted by a coffee shop that Daigo's father had operated before he ran off with a waitress when Daigo was six; since then the two have had no contact. Daigo feels hatred towards his father and guilt for not taking better care of his mother.
