Il s'agit du quatrième épisode de la série Les Aventures de Sherlock Holmes et du docteur Watson réalisée par Maslennikov.
Le film se compose de deux épisodes d'environ 1 h chacun. L'intrigue est une adaptation fidèle du roman Le Signe des quatre d'Arthur Conan Doyle, ainsi que de la nouvelle Un scandale en Bohême. Les deux intrigues ne sont pas mélangées : Holmes enquête sur le cas du roi de Bohême au début du second épisode du téléfilm, alors que l'enquête du Signe des quatre est momentanément en suspens.
Le duo Holmes / Watson est incarné par Vasili Livanov et Vitali Solomin.

Sherlock Holmes (Basil Rathbone) pretends to be a Nazi spy to aid scientist Dr. Franz Tobel (William Post Jr.) and his new invention, a bombsight, in escaping a Gestapo trap in Switzerland. Holmes and Franz fly to London, where Holmes places him under the protection of his friend, Dr. Watson (Nigel Bruce). The scientist slips away against Holmes' instructions for a secret reunion with his fiancee, Charlotte Eberli (Kaaren Verne), and gives her an envelope containing a coded message. He tells Charlotte to give it to Holmes if anything should happen to him. Leaving Charlotte's apartment, an attempt to abduct him by German spies is foiled by a passing London bobby.

The film is divided into two separate, unequal stories. In the shorter of the two, Holmes is approached by a famous Russian ballerina, Madame Petrova (Tamara Toumanova), who proposes that they conceive a child together, one who she hopes will inherit her physique and his intellect. Holmes manages to extricate himself by claiming that Watson is his lover, much to the doctor's embarrassment.

In 1891, Irene Adler delivers a package to Herr Dr. Hoffmanstahl; payment for a letter he was to deliver. Hoffmanstahl opens the package, triggering a hidden bomb that is prevented from detonating by the intervention of Sherlock Holmes. Holmes takes the letter while Adler and Hoffmanstahl escape. Holmes finds Hoffmanstahl assassinated moments later. Adler meets with Professor Moriarty to explain the events, but Moriarty poisons and kills her — deeming her position compromised by her love for Holmes.
