Synopsis
Dr. Alan Feinstone is a successful dentist. However, everything changes on the day of his wedding anniversary, when he discovers his wife Brooke is cheating on him with the poolman, Matt. After they finish, Alan retrieves his pistol and follows Matt in his car. He is led to Paula Roberts's house, a friend of Brooke's. Alan invites a story about surprise party for Brooke and watches Paula invite Matt inside. Paula's dog attacks Alan, and he shoots it in self-defense. After returning to his car, he drives to work.Actors

Dr. Alan Feinstone is in the maximum security mental hospital he was sentenced to at the end of the first film. While talking to the facility's psychiatrist, he remembers the murders he committed in his own mind, while convincing the doctor that it was another man who did those things. His remorseful story distracts her from seeing him pull a sharpened tool that he stitched into his own leg, and he uses her as a hostage to escape the hospital. Alan's ex-wife Brooke is alive despite her missing tongue and inability to speak (She has since had new dental implants put in to replace all the teeth that Alan pulled out in the first film); she hires an investigator to find out where Alan has escaped to, believing that he had been putting away money before he went crazy. Brooke has in her possession some postcards that Alan had left behind, and she believes he is in one of those places.

For the past 13 years, Dr. Herbert West (Jeffrey Combs) has been serving a prison sentence due to a murder at the hands of one of his zombies. With what scant supplies he has on hand in the prison medical center, Dr. West has been capable of performing only extremely basic experiments on rats. However, his lack of supplies does not prevent him from uncovering a key element in his re-animation process. Dr. West has discovered "NPE" (Nano-Plasmic Energy), an energy that can be extracted from the brain of a living organism through an electrocution-like process, to be stored in a capsule resembling a small light bulb. The capsule can then be connected to a corpse and used in conjunction with West's previously developed reagent to restore the former dead to a lifelike state. The NPE prevents the degeneration seen in previous instances, where the reanimated are nothing more than mindless zombies. Used together with the re-agent, reanimated corpses regain their skills, memories, and motor functions and nearly fully resemble normal humans.

Eight months after the events of Re-Animator, Doctors Herbert West and Dan Cain are working as medics in the middle of a bloody Peruvian civil war. In the chaos of battle and with plenty of casualties to work on, they are free to experiment with West's re-animation reagent. When their medical tent is stormed by the enemy troops, West and Cain return home to Arkham, Massachusetts. There, they resume their former jobs as doctors at Miskatonic University Hospital, and West returns to the basement laboratory of Cain's house to continue his research.

The film begins with Leatherface bludgeoning a young woman, Gina, to death with a sledgehammer before beginning the process of cutting off her face in order to make it into a mask. Gina's sister, Sara watches from a nearby window. Leatherface hears Sara outside, and after she flees, he briefly checks outside before going back to work. Some time later, Michelle and her boyfriend Ryan, a couple that constantly argues, both traveling through Texas, pass through a security checkpoint near the excavation of a large number of butchered bodies, a cop approaches them and interrogates them, shortly after they leave and reach the Last Chance Gas Station. At the station, they meet a hitchhiking cowboy named Tex and the station's perverted owner Alfredo. Tex shows Ryan a route on the map that can get them to a town. A fight soon breaks out between Tex and Alfredo when Tex finds the latter spying on Michelle while she uses the station restroom. As Michelle and Ryan flee from the station in their car, they witness Alfredo apparently killing Tex with a shotgun.

A plague in the form of a toxic viral gas is unleashed at major sporting events across the United States. The gas turns its victims instantly "into an immediate rage of insanity and violence". A pair of Germans visiting a remote Greek island go to the beach, and are slaughtered by someone who emerges from the ocean. On the mainland, five travelers are preparing to tour the islands, and are joined by Julie, who asks for a ride to an island that some friends of hers live on. The only one who objects to this detour to the island (which Julie explains has only a few permanent residents, and only sees tourists a few months out of the year) is Carol, whose tarot cards convince her that something bad will happen if they go to the island. The group sails to the island anyway, and while disembarking the pregnant Maggie hurts her ankle, so she stays behind on the boat with its owner. A man attacks the boat, ripping the sailor's head off, and abducting Maggie.
