A group of terrorists led by Commander Topov (Zahary Baharov) kidnap the Ukrainian prime minister's son and daughter and hold them hostage, demanding the release of their imprisoned comrades within 72 hours. In addition, they have taken over the crippled Chernobyl Nuclear Power Plant and threaten to detonate it if their demands are not met. It is revealed that among the ranks of the terrorists is an experimental Next-Generation UniSol (NGU) (Andrei "The Pit Bull" Arlovski), who was smuggled in by rogue scientist Dr. Colin (Kerry Shale). U.S. forces join up with the Ukrainian army at the plant, but quickly retreat when the NGU slaughters most of them effortlessly. Dr. Porter (Garry Cooper), Dr. Colin's former colleague on the Universal Soldier program, revives four UniSols to take down the NGU, but they are systematically eliminated.

The plot centers on Lundgren's character Nikolai Petrovitch Radchenko, a Soviet Spetsnaz operative who is sent to an African country where Soviet, Czechoslovakian and Cuban forces are helping the government fight an anti-communist rebel movement. He is tasked with the mission to assassinate the rebel leader. In order to infiltrate the rebel movement and get within striking distance of his target, he stirs up trouble in the local bar and gets arrested for disorderly conduct. He is put in the same cell as a captured resistance commander and gains his trust in facilitating the escape. Upon finally reaching the rebel encampment he is met with distrust by the rebels. During the night he attempts to assassinate his target but does not succeed when the distrustful rebels anticipate his actions.

The film's plot revolves around the coming of the Apocalypse, heralded by the imminent liberation of the Antichrist from the depths of Hell through a certain gateway at the close of one full millennium. This gate can only be opened by a special key, which has been kept guarded by the order of the Templars (who in this version existed since the last days of Jesus). The key in turn is sought out by the servant of the Antichrist, simply known as the Minion, a demonic spirit that transfers itself into the next available host body when his previous one is killed off. The film opens in a Warri prison in the Niger Delta where a noose is being set up to hang Ebiere (Mbong Amata). It switches to Los Angeles, California; Tom Hudson (Mickey Rourke), CEO of Western Oil is kidnapped on his way to the airport by a movement group tagged "United People's Front for the Emancipation of the Niger Delta People of Nigeria". The kidnap is carried out by Tamuno (Enyinna Nwigwe - the leader of the movement, Timi (Wyclef Jean), Opuwei (Akon), Timi (Razaaq Adoti) and Pere (Robert Peters) after orchestrating an accident near the second street tunnel. Also kidnapped is a reporter, Kristy (Kim Basinger) and her cameraman. Tom Hudson, his wife (Kristin Peterson) and Kristy, together with several others at the accident scene are captured and held hostage within this tunnel which has already been closed off on both sides by the group. Seven hours, no communication is heard from the tunnel - the police, the anti-terrorism unit and the general public have no idea who the "terrorists" are or what they want. Meanwhile in the tunnel, Tamuno tells Tom that they are in Los Angeles to save Ebiere and declares that if Ebiere is eventually hung in Nigeria, then the "truly guilty" ones should go with her. He releases the women and children and instructs Kristy to record what is happening in the tunnel.

John (Scott Adkins) awakens from a coma to find that his wife and daughter were brutally murdered in a home invasion. A group of mercenaries led by Maj. Max Gatling are fleeing from a failed rescue attempt, having been hired by a rich industrialist to enter a zombie-infested city to rescue his daughter Jude. At the extraction point, Gatling tells Smiley that he is staying to find Jude and complete the mission. After searching through the city and killing many zombies on the way, he locates Jude and tells her that he is going to get her out of the city, and she takes him to other survivors. There, he is introduced to the survivors including group leader Duke, Reese, Elvis, Lynn, and Anna. Jude is revealed to be in a relationship with Reese and pregnant with his child. The next day when Gatling, Jude, and the others go out to retrieve gasoline from a gas station, Gatling attempts to get Jude out of the city by force when she refuses to go without the others. However, he is stopped by the others and left for dead. After discovering that Jude is pregnant, Reese rushes to Gatling's aid so that Jude can be taken out of the city. At this point it is revealed that not only is Jude's father some rich industrialist, but he is also the one who caused the zombie pandemic. While returning to the stronghold to get Jude, Gatling and Reese discover a band of non-programmed robots. Gatling reprograms the robots and proceeds to the stronghold. When it is revealed that the city is to be firebombed, Gatling and all of the survivors make preparations to escape.
