Edward Lewis (Richard Gere), a successful workaholic businessman and corporate raider in Los Angeles on business, accidentally takes a detour on Hollywood Boulevard while looking for Beverly Hills. Peter Banning is a successful corporate lawyer who spends more time at work than with his wife Moira and children Jack and Maggie. The Bannings fly to London to visit Moira's grandmother, an older Wendy Darling, to celebrate her charity work for orphans, which once included Peter. During the visit, Peter is distracted by phone calls from his office and on one occasion, shouts at his children when they interrupt him, leading Moira to throw his cellphone out of an open window. Later, while Peter, Moira, and Wendy attend a banquet ceremony hosted by Great Ormond Street Hospital, a strange presence abducts Jack and Maggie. In the 1990s, U.S. National Security Agency official Thomas Bryan Reynolds (Jon Voight) meets with congressman Phil Hammersley (R-NY) (Jason Robards) in a public park to discuss support for new counter-terrorism legislation the U.S. Congress is pushing that dramatically expands the surveillance powers of intelligence agencies on individuals and groups. Hammersley remains committed to blocking its passage, since he believes it would almost totally destroy the privacy of American citizens. Reynolds has his team murder Hammersley, spread heart pills over his car, place him in the car and push it in a lake to make it look like he had a heart attack. In the aftermath, they discover too late that wildlife researcher Daniel Zavits (Jason Lee) had a camera aimed in the woods at their location. Zavits inspects the footage, transfers the video to an innocuous video game disc and flees the apartment ahead of Reynolds's team.

In January 2000, Dick Harper (Jim Carrey) has just been promoted to Vice President of Communications for the large media corporation Globodyne. The next day, he is asked to appear on the show Money Life, where host Sam Samuels and independent presidential candidate Ralph Nader dub him and all the company's employees as "perverters of the American dream" and claim that Globodyne helps the super rich get even wealthier. As they speak, the company's stock goes into a free-fall and is soon worthless, along with all the employees' pensions, which are in Globodyne's stock. Dick arrives home to find his excited wife Jane (Téa Leoni), who informs him that she took his advice and quit her job as a travel agent after his promotion in order to spend more time with their son Billy. Dick has to break the news over dinner, instantly alarming Jane. Dick tries to think positively, and tries for a few months to get a Vice Presidency at other corporations. Famed novelist Paul Sheldon (James Caan) is the author of a successful series of Regency romance novels featuring a character named Misery Chastain. Wanting to focus on more serious stories, he writes a manuscript for the final Misery novel. While traveling from Silver Creek, Colorado to his home in New York City, he is caught in a blizzard and his car goes off the road, rendering him unconscious. Paul is rescued by a nurse named Annie Wilkes (Kathy Bates), who brings him to her remote home. When Paul regains consciousness he finds himself bedridden, with both his legs broken as well as a dislocated shoulder. Annie claims she is his "number one fan" and talks a lot about him and his novels. As a reward for saving him, Paul gives Annie his new manuscript which she saved from the wreckage. While feeding him, she is angered and spills soup on him but regains control and apologizes. She buys a copy of Paul's most recently published book, Misery's Child, giving glowing praise to Paul as she progresses through the book. However, when Annie discovers that Misery dies at the end of the book she flies into a rage, almost smashing a table on Paul's head. She reveals that she lied about calling his agent and the authorities; nobody knows where he is. Annie leaves and Paul tries to escape from his room, but she has locked the door.

Dick arrives home to find his excited wife Jane (Téa Leoni), who informs him that she took his advice and quit her job as a travel agent after his promotion in order to spend more time with their son Billy. Dick has to break the news over dinner, instantly alarming Jane. Dick tries to think positively, and tries for a few months to get a Vice Presidency at other corporations. After getting no job anywhere, Jane reveals that they'll end up declaring bankruptcy in the next couple of months due to their assets being made up entirely of now-worthless Globodyne stock.

Recently divorced Meg Altman (Foster) and her 11-year-old daughter Sarah (Stewart) have just purchased a four-story brownstone on the Upper West Side of New York City. The house's previous owner, a reclusive millionaire, installed an isolated room used to protect the house's occupants from intruders. The "panic room" is protected by concrete and steel on all sides, a thick steel door, and an extensive security system with multiple surveillance cameras, a public announcement system, and a separate phone line. On the night the two move into the home, it is broken into by Junior (Leto), the previous owner's grandson; Burnham (Whitaker), an employee of the residence's security company; and Raoul (Yoakam), a ski mask-wearing gunman recruited by Junior. The three are after $3 million in bearer bonds, which are locked inside a floor safe in the panic room.

Sam Dawson (Sean Penn), a man with a developmental disability, is the single father of Lucy (Dakota Fanning), following their abandonment by her mother, who is revealed to be a homeless woman who "just needed a place to sleep". Despite his limitations, Sam is well-adjusted and has a supportive group of friends with developmental disabilities, as well as a kind, agoraphobic neighbor Annie (Dianne Wiest) who takes care of Lucy when Sam cannot. Though Sam provides a loving and caring environment for precocious Lucy, she soon surpasses his mental ability. Famed novelist Paul Sheldon (James Caan) is the author of a successful series of Regency romance novels featuring a character named Misery Chastain. Wanting to focus on more serious stories, he writes a manuscript for the final Misery novel. While traveling from Silver Creek, Colorado to his home in New York City, he is caught in a blizzard and his car goes off the road, rendering him unconscious. Paul is rescued by a nurse named Annie Wilkes (Kathy Bates), who brings him to her remote home. When Paul regains consciousness he finds himself bedridden, with both his legs broken as well as a dislocated shoulder. Annie claims she is his "number one fan" and talks a lot about him and his novels. As a reward for saving him, Paul gives Annie his new manuscript which she saved from the wreckage. While feeding him, she is angered and spills soup on him but regains control and apologizes. She buys a copy of Paul's most recently published book, Misery's Child, giving glowing praise to Paul as she progresses through the book. However, when Annie discovers that Misery dies at the end of the book she flies into a rage, almost smashing a table on Paul's head. She reveals that she lied about calling his agent and the authorities; nobody knows where he is. Annie leaves and Paul tries to escape from his room, but she has locked the door.
