Carolco Pictures, Inc. was an American independent film production company that, within a decade, went from producing such blockbuster successes as Terminator 2: Judgment Day, Total Recall, and the first three movies of the Rambo series (First Blood, Rambo: First Blood Part II, and Rambo III) to being bankrupted by box office bombs such as Cutthroat Island and Showgirls.

John Connor has been living off the grid in Los Angeles in the years following the death of his mother, Sarah, from leukemia. Although Judgment Day did not occur on August 29, 1997, as expected, John still believes that a war between humans and the machines will occur. Unable to locate John in the past, Skynet sends a new model of the Terminator called the T-X back to July 24, 2004 to kill other members of the Human Resistance. The T-X is more advanced than previous Terminators, and has an endoskeleton with built-in weaponry, a liquid metal exterior similar to the T-1000, and the ability to reprogram other machines. Unlike previous Terminators, its standard appearance is female. The Resistance sends a reprogrammed Terminator (T-850 - Model 101) back in time to protect the T-X's targets including John and his future wife, Kate Brewster.

Nomi Malone is a young drifter who hitchhikes to Las Vegas hoping to make it as a showgirl. After being cheated of her money by her driver, Nomi meets Molly Abrams, a seamstress and costume designer who takes her in as a roommate. Molly invites Nomi backstage at Goddess, the Stardust Casino show where she works, to meet Cristal Connors, the diva star of the topless dance revue. When Nomi tells Cristal she dances at Cheetah's Topless Club, Cristal derisively tells her that what she does is akin to prostitution. When Nomi is too upset to go to work that night, Molly takes her dancing at The Crave Club. After getting into a fight with James, a bouncer at the club, Nomi is arrested. James bails her out of jail, but she pays him little notice.

In 1668 Jamaica, Morgan Adams hunts down her uncle and fellow pirate Dawg Brown, who has captured her father, Black Harry. Black Harry has one of three pieces of a map to a huge stash of gold on the remote Cutthroat Island. Dawg has another piece, having stolen it from the corpse of a third brother, Richard, while a fourth brother, Mordechai, has the last piece. Harry refuses to give Dawg his piece and escapes with Morgan's help, but not before being mortally wounded. A dying Harry reveals to his daughter the location of the map piece: on his scalp.

Distraught but skillful bounty hunter Lewis Gates (Tom Berenger) pursues three armed escaped convicts into Montana's Oxbow Quadrangle, followed by his faithful companion called Zip, an Australian cattle dog at the persistence of his unforgiving father-in-law, whom blames Gates for his daughter's tragic death. All Gates finds is a few scraps of cloth, some blood, and an old-fashioned Indian arrow. He takes the arrow to anthropologist Lillian Sloan (Barbara Hershey) who identifies it as a replica of the arrows used by Cheyenne Dog Soldiers. Gates doesn't think it's a replica and, after some library research, develops a long list of people who have disappeared into the Oxbow and a story of a "wild child" captured in the woods in the early 20th century. Now he's convinced that the fugitives were killed by a tribe of Dog Soldiers, a hardy band of native Americans who had somehow escaped the 1864 Sand Creek massacre and survived for 128 years secluded in the Montana Wilderness, killing anyone who threatened to find and expose them.

In the 1860s Wild West, a group of misfit settlers including ex-doctor Phil Taylor (Lewis), prostitute Belle (Ellen Greene), and homosexual bookseller Julian (John C. McGinley) decide they cannot live in their current situation in the west, so they hire a grizzled alcoholic wagon master by the name of James Harlow (Candy) to take them on a journey back to their hometowns in the East. This leads to comedic exploits when the drunken wagon master leads them into Sioux territory and they are pursued by the cavalry. They also have to contend with hired gunslingers who have been sent by railroad magnates to stop the journey in fear of bad publicity of the west, and their discovery that Harlow had been part of the infamous Donner Party.

A massive metal ring with engraved cover stones is discovered in the sands of Giza, Egypt in 1928, by an archaeologist named Langford and his young daughter, Catherine. Jackson accepts and travels to a US Air Force installation where he examines the stones. A retired rock star, Johnny Boz, is stabbed to death with an ice pick during sex by a mysterious blonde woman at his apartment. Homicide detective Nick Curran investigates, and the only suspect is Catherine Tramell, Boz's bisexual girlfriend and a crime novelist who has written a novel that mirrors the crime. It is concluded that Catherine herself did it or someone who is trying to frame her out of spite. Tramell is uncooperative and taunting in the investigation, smoking in the interrogation room and exposing her bare genitalia in front of the officers. She presents alibis and passes a lie detector test. Nick discovers that Catherine has a habit of befriending murderers, including her girlfriend Roxy, who is later shown to have murdered several young boys on impulse, and Hazel Dobkins, who murdered her family.

Then, upon hearing that an old friend named Ramon Morales was killed in a crash in the Gulf of Mexico, Chappy is summoned to Lethridge Air Force Base in Brownsville, Texas, where the remains of Ramon's plane are being examined. Chappy mentions that among Ramon's surviving family members are his sister Anna, who graduated from UCLA on an athletic scholarship, and his father, the mayor of a small Peruvian village. It is discovered that Ramon was shot down while carrying several kilograms of cocaine, which places this case under DEA jurisdiction.

In 1969, an American US Army Special Forces team receives orders to secure a village against North Vietnamese forces. Private Luc Deveraux (Jean-Claude Van Damme) discovers members of his squad and various villagers dead, all with their ears removed. Deveraux finds Sergeant Andrew Scott (Dolph Lundgren), who has gone insane, with a string of severed ears and holding a young boy and girl hostage. Devereaux, who is near the end of his tour of duty, tries to reason with Scott, who shoots the boy and orders Devereaux to shoot the girl to prove his loyalty. Deveraux refuses and tries to save the girl, but Scott kills her with a grenade. The two soldiers shoot each other to death. Deveraux and Scott's corpses are recovered and iced by a second U.S. Special Forces squad, their deaths covered up as "missing in action.

In 1995, John Connor is ten years old and living in Los Angeles with foster parents. His mother Sarah Connor had been preparing him throughout his childhood for his future role as the leader of the Human Resistance against Skynet, but was arrested after attempting to bomb a computer factory and imprisoned at a mental hospital under the supervision of Dr. Silberman. Skynet sends a new Terminator, designated as T-1000, back in time to kill John. The T-1000 is an advanced prototype composed of a mimetic polyalloy that allows it to take on the shape and appearance of almost anything it touches, and transform parts of its anatomy into knives and other stabbing weapons. The T-1000 arrives under a freeway, kills a policeman and assumes his identity. Meanwhile, the future John Connor has sent back a reprogrammed T-800 (Model 101) Terminator to protect his younger self.
