Synopsis

In Morocco, Frank Cotton buys a puzzle box from a dealer. In a bare attic, Frank solves the puzzle, and hooked chains emerge from it to tear into his flesh. Later the room is filled with swinging chains spattered with the remnants of his body. A black-robed figure picks up the puzzle box and returns it to its original state, restoring the room to normal.

In Romania, theology student Allison has come into possession of The Lexicon, a mysterious book of prophecies that writes itself. This book contains a 23rd chapter of the book of Revelations, which is still not complete. The last chapter depicts the end of the war of angels and the name of the Antichrist. One of the angels who fell with Satan, Belial (now a demon), wants this book. Simon, a good angel, opposes him and guides Allison by taking advantage of her mental illness to speak directly to her. While searching for Allison and the book, and to avoid detection, Belial murders people and takes their form.

The film introduces a circle of youths who are addicted to playing Hellworld, an online computer game based on the Hellraiser series. The film opens at the funeral of Adam, one of the friends who was obsessed with the game and ultimately committed suicide after becoming too immersed in the game. Joseph Thorne is an intelligent yet corrupt Denver police detective who regularly indulges in drug use and infidelity during the course of duty. At the scene of what appears to be a ritual murder, Thorne discovers a strange puzzle box, which he takes home in order to indulge his fascination with puzzles. After solving the box, Thorne begins to experience bizarre hallucinations, such as being seduced by a pair of mutilated women and being chased by a creature with no eyes or legs. Thorne also makes a connection between the murder and a killer known as "The Engineer," who is suspected of having kidnapped a child. The film opens with a dream sequence in which stock market tycoon Paul Marsh discovers a mermaid with razor sharp teeth while scuba diving into a carven pit. Paul awakes on a boat off the shores of Spain, where he is vacationing with his girlfriend, Barbara, and their friends Vicki and Howard. A sudden storm blows their boat against some hidden rocks. Vicki is trapped below deck, and Howard stays with her while Paul and Barbara take a lifeboat to the nearby fishing village of Imboca. During their absence, an unseen creature from the deep attacks the two in the boat.
