A sting operation to capture notorious arms dealer Miles Jackson (Aidan Gillen) goes awry when the FBI's inside man double crosses them. Miles rendezvouses with his girlfriend Erica Kessen (Taylor Cole) who has a getaway car. Officer Danny Fisher (John Cena) and his partner, Officer Hank Carver (Brian J. White), are dispatched to help the FBI. On their way, they look at Jackson's record and see a surveillance video of him dancing with Kessen. A sudden encounter with them when they spot her at a traffic light leads to her death and Miles being taken away. Miles swears vengeance on Fisher.

The film opens with a bloodied and terrified priest slowly making his way across the bodies of thousands of dead soldiers. There are many crows and hyenas roving around the bodies. The priest reaches the dead body of another priest and tries take a small demon idol of the head of Pazuzu from his hand, but, suddenly, the dead priest briefly comes back to life and stops the living priest from taking it. The camera pulls back to reveal that the entire valley is littered with dead soldiers, many have been crucified upside down. The movie then cuts to Cairo, Egypt in 1949, where the young Father Lancaster Merrin (played by Skarsgård, who played the same part in Dominion: Prequel to the Exorcist) has taken a sabbatical from the Church and devoted himself to history and archaeology as he struggles with his shattered faith. He is haunted especially by an incident in a small village in the occupied Netherlands during World War II, where he served as parish priest: near the end of the war, a sadistic Nazi SS commander, in retaliation for the murder of a German trooper, forces Merrin to participate in arbitrary executions in order to save a full village from slaughter.

At Aquatica, a remote former submarine refueling facility converted into a laboratory, a team of scientists is searching for a cure for Alzheimer's disease. Fluids from the brain tissue of three Mako sharks are being harvested as a cure for Alzheimer's. As the Californian coastal town of Antonio Bay is about to celebrate its 100th anniversary, paranormal activity begins to occur at the stroke of midnight. Town priest Father Malone is in his church when a piece of masonry falls from the wall, revealing a cavity containing an old journal, his grandfather's diary from a century ago. It reveals that in 1880, six of the founders of Antonio Bay (including Malone's grandfather) deliberately sank and plundered a clipper ship named the Elizabeth Dane. The ship was owned by Blake, a wealthy man with leprosy who wanted to establish a leper colony nearby. Gold from the ship was used to build Antonio Bay and its church.

A recently promoted police inspector (Gian Maria Volonté) kills his mistress (Florinda Bolkan), and then covers up his involvement in the crime. He insinuates himself into the investigation, planting clues to steer his subordinate officers toward a series of other suspects, including the woman's gay husband and a student radical. He then exonerates the other suspects and leads the investigators toward him, in order to prove that he is "above suspicion" and can get away with anything, even while being investigated. In the end, he confesses to the crime in front of his superiors - who refuse to believe him. Sure that he is safe, he recants his confession, and receives the approval of the police commissioner.
