Synopsis
The film begins with two men on their motorcycles driving away from Pumpkinhead. One of the men hits a tree branch in their path, falling from his motorcycle and allowing Pumpkinhead to catch up to him. As the man is being killed, the film cuts to a man in a log cabin who seems to share the pain inflicted by Pumpkinhead on the fallen man. The surviving man, named Dallas, rides to the log cabin, and the man who conjured Pumpkinhead, begging him to call the demon off. Pumpkinhead smashes through the window and Dallas attempts to fend him off by shooting him with a small pistol with little effect, and is clawed in the chest by the demon. When Dallas realizes that his bullets have no effect on Pumpkinhead, he swears to take the summoner with him, shooting the man and killing him, causing Pumpkinhead to vanish. Ed Harley then appears telling Dallas that Pumpkinhead will return and there will be no place to hide.Actors

Amy is an elementary school teacher who suffers from nightmares about doctors and hospitals, stemming from the circumstances of her father's death. On the afternoon of the day he proposed, Amy and her fiancé Nick are in a serious car accident. An ambulance takes Nick away without telling Amy which hospital they are going to. Amy and Lucas, the driver of the truck that hit them, try to find where Nick and Lucas' sister have been taken. Amy begins to hallucinate, seeing disfigured faces, including her own in a mirror. A little girl in her class, Melissa, claims she can help Amy find Nick, and gives Amy the name of St. Rosemary's Hospital. Amy and Lucas learn that St. Rosemary's, which was rumored to be the home of devil worshipers, was destroyed in a fire some 70 years ago, along with all the nurses and staff, who refused to leave. Meanwhile, Nick is having his own strange experiences at St. Rosemary's, such as nurses who spray blood on each other and eat another patient, and a hallway that repeats itself.

Taking place shortly after the original, a professor at the fictional Cuesta Verde University has managed to subdue and contain a "Hyper sapiens" specimen, who is revealed to be an infected survivor from the first film. The professor experiments on the survivor, trying to determine the source of her immortality, with apparent disregard for the rotted state of her body. When he has created a serum that he believes will bring back the dead and grant immortality, he murders one of his students and injects her with the serum. She returns to life, infects the professor, and breaks out of the building.

The mini-series features the bloody history of the House of Batiatus and the city of Capua before the arrival of Spartacus. De nos jours, Hank Holten, un inspecteur de police mal noté par ses supérieurs depuis qu'il est divorcé est chargé d’enquêter sur la disparition mystérieuse d'une jeune femme. La police ne pssoède aucune piste et Hank commence son enquête dans les bars et les boites de nuit muni de la photo de la jeune disparue. Croyant la reconnaître dans une boite de nuit, il tente de l'aborder malgré l'opposition d'un homme qui se présente comme son frère. Cependant la jeune femme Leyla n'étant pas insensible aux charmes du policier l’entraîne dans une camionnette et lui pratique une fellation en présence du "frère", lui-même occupée avec une autre jeune femme. Leyla conduit alors Hank a une rave party dans un acier hôpital désaffecté. Hank se retrouve alors au milieu d'une véritable orgie et sa volonté semble annihilée. Leyla souhaite faire l'amour avec lui sans aucun préliminaire. Après quelques minutes, une partie des participants révélé leur véritable nature, ce sont des vampires, et ces derniers se mettent à mordre le cou des personnes qui se sont laissées entraîner. Quand Leyla veut à son tour mordre Hank, celui-ci a un sursaut et tente de s'échapper, mais il est rattrapé par le chef hiérarchique de la secte qui le mord au cou.

Pumpkinhead: Ashes to Ashes plotline follows more closely to the first film, with townspeople angered over the local mortician stealing and selling the organs of their loved ones and then dumping the corpses in a swamp, rather than cremating them. When the townspeople find out, they have the old witch Haggis summon Pumpkinhead through the mummified body of Ed Harley (played by Lance Henriksen, who reprises his role from the first film). Pumpkinhead then proceeds to go on his killing rampage murdering all those responsible for the desecration, while Doc Frasier (played by Hellraiser's Doug Bradley) hurries to murder those who summoned Pumpkinhead, which will effectively kill the demon in the process.
