Abdul Razaq (Siddique) and Fareeda (Praveena), a Muslim couple in Kerala, are expecting their first child. Razaq wants the child to be a boy, and wants to name the child Faizal and call him Faizi, but to his disappointment, the child is a girl. Fareeda gives birth continuously to three more girls, which irritates Abdul Razaq, who migrates to Dubai. Finally, Fareeda gives birth to a boy, but because of her continuous deliveries, Fareeda becomes ill and dies. Abdul Razaq takes his five children to Dubai. Faizi is looked after by his sisters whom he affectionately calls "Ithathas & Company" (Sisters and Company).

Annan Thambi is the story of twin brothers, Appu and Achu (both played by Mammootty). Appu is a talkative conman who always finds himself in trouble. While Achu, a mute, is nicer than his brother, he draws a dagger on Appu due to misunderstandings created by the villain Govindan (Siddique), who is masquerading as his friend. Their father Ravunni (Janardhanan), a ballet artist, separates them due to sibling rivalry, and Appu is sent to live with relatives in Pollachi.

In 2000, George (Nivin Pauly), a 16-year-old pre-degree student, develops an unrequited love for Mary (Anupama Parameshwaran), the so-called "prettiest girl" in the region of Aluva. George and his friends Shambu (Shabareesh Varma) and Koya (Krishna Shankar) follow Mary and her companion Celine (Eva Prakash) around every day. The film's story revolves around three cousins Divya (Nazriya Nazim), Krishnan P.P., also known as Kuttan (Nivin Pauly) and Arjun (Dulquer), who share a close bond since their childhood.
