As the Californian coastal town of Antonio Bay is about to celebrate its 100th anniversary, paranormal activity begins to occur at the stroke of midnight. Town priest Father Malone is in his church when a piece of masonry falls from the wall, revealing a cavity containing an old journal, his grandfather's diary from a century ago. It reveals that in 1880, six of the founders of Antonio Bay (including Malone's grandfather) deliberately sank and plundered a clipper ship named the Elizabeth Dane. The ship was owned by Blake, a wealthy man with leprosy who wanted to establish a leper colony nearby. Gold from the ship was used to build Antonio Bay and its church.

In a flashback, Deborah Myers (Sheri Moon Zombie) visits her son, a young Michael Myers (Chase Wright Vanek), at Smith's Grove Sanitarium. She gives him a white horse statuette as a gift. Michael says that the horse reminds him of a dream he had of Deborah's ghost, dressed in all white and leading a horse down the sanitarium halls toward Michael, telling him she was going to bring him home. Fifteen years later, after having shot an adult Michael (Tyler Mane), Laurie Strode (Scout Taylor-Compton) is found wandering around in a state of shock by Sheriff Brackett (Brad Dourif), who takes Laurie to the emergency room. Meanwhile, the paramedics pick up the Sheriff's daughter and Laurie's friend Annie (Danielle Harris) and Michael's psychiatrist Dr. Loomis (Malcolm McDowell), who are still alive after having been attacked by Michael, and take them to the hospital. Presumed dead, Michael's body is loaded into a separate ambulance. When the driver has a traffic accident, Michael awakens and escapes the ambulance, walking toward a vision of his mother dressed in white and leading a white horse.

William Blake arranges to purchase half of Antonio Island, off the coast of Oregon, to establish a leper colony for his people. Twenty years after the events of Halloween and Halloween II, on October 29, 1998, Marion Chambers (Nancy Stephens), Dr. Sam Loomis' former colleague, returns to her house in Langdon, Illinois, to find it has been burglarized. Her teenage neighbor Jimmy (Joseph Gordon-Levitt) searches the house and finds nothing. While waiting for the police in her house, Marion discovers a file is missing, the one on Laurie Strode (Jamie Lee Curtis), Michael Myers' sister who escaped the attacks two decades earlier. The movie takes place in the 1950s. Sabrina Masterson is a conceited and wealthy teenager who is sent by her own mother to college to live at Alpha Beta Pi, a sorority house, because she couldn't handle her any more, explaining she was too evil. Upon her arrival, she is welcomed by everyone and assigned as the roommate of Rita Summers, the sorority president and most adored and popular girl. On October 31, 1989, Michael Myers and his niece Jamie Lloyd (Danielle Harris) have been abducted by a mysterious stranger in black at the Haddonfield Police Station. Six years later, teenager Jamie (J. C. Brandy) has been impregnated and her baby is born on Halloween Eve. The film begins with the conclusion of the previous film after Michael Myers (Don Shanks) fell down a mine shaft. The state troopers tossed dynamite down the mine to make sure that Michael was dead. But Michael escapes and stumbles into a nearby river, where he is found by a hermit. Michael falls into a coma for one year while in the hermit's care. On October 30, 1989, Michael awakens, kills the hermit, and returns to terrorize Haddonfield, where his young niece Jamie Lloyd (Danielle Harris) continues to live after nearly being killed by Michael the year before.
