K. Ramachandra Babu is a Kerala State award-winning cinematographer of over 125 films, out of which most are Malayalam films. He has also worked for films in Tamil, Telugu, Hindi, Arabic and English languages. He has served as the cinematographer for several documentaries and advertisement films as well.

Bharathan is a post-graduate and unemployed. Somehow he manages to get the job of a Malayalam teacher at a tuition centre. His wife Geetha works in a consumer store. The couple stays in a rented house owned by Kariyachan. Bharathan is a genius inventor and has a rather inquisitive mind especially as far as scientific matters are concerned. He is always around inventions with little care on what happens around him. He never sticks to any particular job, which lands his family in deep financial troubles. His wife Geetha is upset about all this, but little could she do to avoid her husband from becoming the laughing stock of many who don't understand him. Some people like Kariachan (Jagathy Sreekumar) cleverly usurp the patent rights of some of Bharathan's crude inventions. The few people who come with helping hands are Thankachan, one of his students in the tuition centre and Peter, who goes around finding him new jobs. In the meantime Bharathan invents a gadget that could be used to clean and sweep the courtyard and also to pluck coconuts and fruits from palms and trees.

The film is set in 16th-century northern Kerala. The plot unfolds at Puthooram, the house of great Kannappan Chekavar (Balan K. Nair). Selina Joseph (Suparna) a budding poet leading a very happy family life, commits suicide on a very 'usual' day by shooting herself with her husband's gun. Upon request from Mathew (Sukumaran), her husband, Balachandran Nair (Mammootty), his friend, a Delhi based journalist decides to look into the reasons that led her to suicide. Balu unfolds the truth behind Selina's death with the help of her childhood friend Shyamala Menon (Parvathy). Selina happened to become pregnant in her childhood following a gang rape. But what makes the film different is that Selina is unaware of this incident. She did not accept the fact even after the birth of her baby. She considered herself as a virgin and called her son Immanuel meaning the son of Holy virgin Mary. Her child was moved to an orphanage by her father Antony (Karamana Janardanan Nair). She lost her memory after losing the child. But on seeing her son again years after, she is terribly distracted and commits suicide. The film has a lot of memorable scenes including the climax scenes, when Shyamala comes to know that she and Selina were raped.

Kaimal and Susheela are leading a troubled married life. The latter is unsatisfied with the ignorance from her husband. Susheela, a mother of two children, indulges in a physical relation with a car mechanic named Giri. On an unfortunate day, she elopes with Giri to start a new life. After the incident, Kaimal is harassed by many of his friends and colleagues. Balan, a friend of Kaimal, helps him to get a relief. Balan tries to bring back Susheela, who was still having an unsatisfied life with Giri, to Kaimal's life. He arranges a meeting between Susheela and Kaimal in a beach. But, they found Kaimal dead after being stabbed by himself with a knife.

Pagal Nilavu is in essence the story of Selvam(Murali) an aimless youth in Muttam(Kanyakumari Dist) who out of a sense of deep gratitude ends up joining the gang of Devaraj aka Periyavar(Sathyaraj) and the complications which arise when he falls in love with Jyothi(Revathi), the sister of Robert Manohar(Sarat Babu) an honest and committed police inspector who is newly posted to the town. Periyavar pretty much runs the town and though he helps people in need, he is a diabolical kingpin who will not compromise on his hold of the town for anything. He wins Selvam’s undying loyalty when he helps him with money and power at a critical juncture to save his mother. In this backdrop Robert takes charge and moves into the town with his sister and daughter. Being uncompromising in his morals, Robert is automatically drawn into a game of oneupmanship with Periyavar and his gang. He also consequentially cannot stand Periyavar’s chief henchman Selvam, and is consequentially drawn into continuous verbal and physical duels with him. Selvam meanwhile falls hook line and sinker to Jyothi(Revathi) and tries every trick in the book to woo her and eventually wins.
