L.A. Detective Sam Dietz (Leo Rossi), struggling to emotionally survive his previous big case (from in the first Relentless film) is unwillingly paired with a shady FBI agent Kyle Valsone (Ray Sharkey), during a case tracking another serial killer (Miles O'Keefe), who kills seemingly at random. Gangster boss Vic Luca (Rip Torn) is scheduled to appear in court and so hires a hit man/shoe salesman Chris Caleek (Lance Henriksen) to kill the witnesses. He has a mole in the police force who tells him names and locations of the witnesses. Unfortunately, during the last hit, the professional killer enters the wrong house. When owner Jack Collins comes home, he finds his pregnant wife unconscious in the kitchen, his friend dead in the living room and his son kidnapped. Wanting Luca to believe he has the real witness' son, Collins is taken into custody. But Collins manages to escape and takes things into his own hands.

The film follows the story of Jennifer Hills (Butler), continuing the story of the first story and not the second; this is the true sequel. After Jennifer survived her assault and exacted revenge on the men that hurt her, she has returned to city life, completely scarred from the whole ordeal. She's no longer an aspiring writer and now devotes herself between working as an assault hotline operator, going to group counselling, and getting personal therapy for her ordeal, under the assumed name, Angela. Because of her mental/emotional scars, she has become highly aggressive and defensive around her environment; she doesn't trust men and presume all simply wants to have her, including the good ones. However, she began developing a bond with one of the girls in the group, Marla, and two began their own personal crusade, by exacting justice for the victims they know. However, just as Jennifer began to enjoy life, Marla died. Distraught and enraged how the law couldn't exact justice for Marla nor the rape victims in her support group, she decided to get revenge for them. One-by-one, Jennifer stalked and lured the scot-free rapists to a private place to torture and kill them. However, her actions began to draw attention from the cops as they suspect she's behind recent murders; she was never caught. Despite the police keeping an eye on her, she managed to evade detection, but she has become further disillusioned with society and enraged there's no justice for wrongdoers. Jennifer was ultimately stopped after she lost her sanity to hurt another man and the police stopped her in the act. Despite being sent to jail and continuing her psychotherapy, Jennifer has become a highly violent and cynical person, unwilling to stop exacting her murderous rage upon those who earned her wrath.

Katie Carter (Dallender) is an aspiring model from Missouri who works as a waitress in a New York City restaurant. Desperate to update her modeling portfolio, she answers an advertisement offering a free photography session. She then meets three Bulgarian siblings, photographer Ivan (Absolom) and his assistants Nikolai, known as "Nicky" (Aleksiev) and Georgy (Baharov), who becomes infatuated with Katie. Pendant la Seconde Guerre mondiale, le projet Philadeplhia Experiment a pour but de rendre les navires de la flotte américaine invisibles à ses ennemis. Le navire Eldrige tente l’expérience et disparaît littéralement, avant de revenir vidé de son équipage. En 2012, une équipe de scientifiques reprennent le projet d'invisibilité et l'essaie sur une automobile. C'est alors que, non loin de Philadelphie, l'Eldrige réapparaît sur une piste d’atterrissage avec à son bord un Navy, Bill Gardner. Celui-ci vient de faire un bond de 70 ans dans l'avenir. Instable, le bateau se téléporte à quelques reprises à travers le monde (Philadelphie, Chicago, Nevada...), ce qui préoccupe le gouvernement qui, bien que désirant cette technologie, ne veut pas laisser fuiter l'existence de tels phénomènes. Alors que celui-ci essaie par tous les moyens de détruire le navire, tuer Gardner et effacer toute trace de cet incident temporel, Bill tente plutôt de retourner en 1943 avec l'Eldrige et ainsi changer le cours de l'histoire.
